超声乳化术后角膜非球面参数与泪膜参数的改变

摘　　要：目的研究白内障超声乳化术后角膜非球面状态与泪膜参数的改变。方法白内障患者102例146只眼分成两组，A组为透明角膜切口组，B组为巩膜切口组。分别于白内障超声乳化术前、术后1d、1周、1月、3月进行角膜地形图，角膜荧光素染色试验，泪膜破裂时间（BUT）检查，并记录角膜离心指数CEI（Central Eccentricity Index，e），BUT值，荧光素试验分值与干眼症状眼数。结果（1）泪膜破裂时间术后1d（8．500±3．477）s、1周（10．966±3．970）s、1月（13．418±3．898）S与术前（15．0±4．318）S比较有明显缩短（P〈0．05），两切口组比较差异无统计学意义；角膜荧光素试验与干眼症状术后1周内变化明显，术后3月基本恢复正常。（2）所有患者角膜CEI值手术后1d、1周与1月与术前比较P〈0．05；角膜切口组与巩膜切口组间对应观察点比较：术后1d差异有统计学意义。（3）角膜非球面参数与泪膜参数的相关性：术后第1d角膜CEI值改变（-0．15±0．19）与泪膜破裂时间变化（-6．49±2．12）s Pearson相关分析，无相关性。结论白内障超声乳化术后角膜非球面形态与泪膜参数早期都会有不同程度的改变，随着切口的愈合渐趋恢复；二者的变化无相关性。Objective To investigate the changes of corneal asphedcal parameter and lacrimal film parameter after cataract phacoemulsification. Methods A total of 102 cases （146 eyes） underwent ultrasonic phacoemulsification were divided into 2 groups, group A （corneal incision） and group B （scleral incision）. The subjective complains of dry eye, tear break-up time, comeal fluorescent staining and corneal map were measured preoperatively and 1 day, lweek, 1 month and 3 months postoperatively. Results BUT time of 1 day （8.500±3.477s）, 1 week （10.966±3.970s） and 1 month （13.418± 3.898s） postoperatively were shorter than preoperative （15.0±4.318s）; corneal fluorescent and dry eye syndromes changed significantly 1 week postoperatively, and went to normal after 3 months. The comeal Central Eccentricity Index （CEI） value changed apparently 1 month postoperatively （P〈0.05）; there was no statistical significance between group A and B, except 1 day posteratively. Pearson correlation between changes of CEI and BUT, no statistical significance was found. Conclusions In the early stage after phacoemulsification, corneal aspherical and lacrimal film parameter changes, no correlation is found between them.
