异体巩膜修补大范围巩膜溶解的疗效观察

摘　　要：目的探索用异体巩膜移植修补大范围巩膜溶解的临床疗效。方法对巩膜扣带术后巩膜大范围缺血性坏死溶解2例（2只眼）和上半部角巩膜葡萄肿穿孔伴眼内炎1例（1只眼）施行同种异体巩膜瓣移植，随访5～12个月。观察患者视力、巩膜植片、视网膜、眼压及其并发症等。结果2例患者视力均较术前提高，1例患者视力与术前相同。巩膜植片无溶解，周围被纤维组织包裹，略呈弧形隆起，并有新生血管长入，形成加厚的“新巩膜”。视网膜在位，眼压正常。结论用异体巩膜移植修补大范围巩膜溶解，较大的植片能与宿眼的巩膜相融合且无排斥反应，能有效维持眼压、眼球外观，尽可能挽救患者的视力。Objective To investigate the therapeutic effect of allogeneic scleral graft to repair large scleral lytic necrosis. Methods Allogeneic scleral transplantation were performed in 2 cases （2eyes） with scleral lytic necrosis after scleral buckling and 1 case （1 eye） with penetrating comeoscleral staphyloma. All cases were followed up for 5 months to 24 months by observing visual acuity, scleral graft, retina, intraocular pressure （IOP） and possible complications. Results Visual acuity were improved in 2 cases, remained in 1 case; the sclearl grafts were wrapped up by vaculized fiber tissue and formed a new thick sclera, no dissolving were found.The retinas were flat, IOP was controlled in normal level. Conclusions Allogeneic scleral graft poor or penetrating sclera and save visual acuity.
