肾癌术后乳糜瘘的临床诊断与治疗分析（附2例报告及文献复习）

摘　　要：目的探讨肾癌根治术后乳糜瘘的诊断与治疗。方法总结2例肾癌术后乳糜瘘患者的临床诊治资料，结合文献讨论其病因发病机理、临床表现、诊断（实验窒及影像学检查）、治疗、预防及处理。结果1例采用保守治疗，1例开放手术腹膜后淋巴管结扎。本组2例患者均治愈，例1术后随访6个月、例2随访8个月，目前均存活，未复发，仍在随访中。结论肾癌术后乳糜瘘I临床比较少见，一旦发生不及时诊治，死亡率极高。确诊须依据临床表现＋实验室＋影像学检查；治疗首选保守治疗，保守治疗无效的情况下可选择腹膜后淋巴管结扎加分流术治疗乳糜外瘘是一种有效的方法。Objectives To discuss the diagnosis and therapy of Renal carcinoma radical correction to incur external chylous fistula. Methods After summarizing 2 example kidney cancer technique, the chyle fistula patient clinical diagnosis material, the union literature discusses its cause of disease pathogenesis, the clinical manifestation, the diagnosis （laboratory and phantom study inspection）, the treatment, the prevention and processing. Resuits 1 example uses the conservative treatment, 1 example opening surgery retroperitoneal lymph vessel ligation. This group of 2 example patient cures, after example 1 technique, makes a follow - up visit 6 months, the example 2 to make a follow - up visit for 8 months, at present survives, has not recurred, still in revisit. Conclusions After kidney cancer technique, the chyle fistula clinical is quite rare, once occurs is not prompt diagnoses, the mortality rate is extremely high. The diagnosis must rest on the clinical manifestation ＋ laboratory ＋ phantom study inspection ; The treatment first choice treats conservatively, in the conservative treatment invalid situation may choose the retroperitoneal lymph vessel ligation to add outside bypass treatment chyle the fistula is one effective method.
