术前术后肠内营养支持对胃癌根治术后患者康复的疗效分析

摘　　要：目的探讨术前术后肠内营养支持对胃癌根治术后患者康复的效果及临床意义。方法将本科收治的68例胃癌患者随机分为三组,对照组(A组)22例,静脉营养支持治疗;术后治疗组(B组)23例,行早期肠内营养;术前治疗组(C组)23例,在早期肠内营养基础上加术前肠内营养支持。观察所有患者术后肛门排气和排便时间;检测三组患者术前4天、术后第1天、第8天患者的体质量、血浆蛋白、前白蛋白、淋巴细胞计数。结果 B组和C组患者术后肛门排气和排便时间均短于A组,差异具有统计学意义(P<0.05);C组患者术后第1天和第8天体质量下降值、血清白蛋白下降值、血清前白蛋白下降值、淋巴细胞计数均明显少于A组和B组,差异均具有统计学意义(P<0.05)。结论术前术后肠内营养支持能明显促进胃癌患者的术后康复。Objective To investigate the comparison of preoperative and postoperative enteral nutrition on efficacy of rehabilitation in postoperative patients with gastric carcinoma. Methods The clinical data of 68 patients with gastric carcinoma admitted our department of gen- eral surgery were analyzed. All the patients were randomly allocated into three groups, in the control group （ Group A）, included 22 patients, patients received only intravenous nutrition postoperatively, in the treatment group of postoperative（ Group B）, patients received only early enteral nutrition postoperatively, In the treatment group of preoperative（ Group C）. Patients received not only preoperative early enteral nutrition but also nutrison by mouth preoperatively for enteral nutrition support. Times of flatus and defecation were observed. The body weight, serum albumin, serum prealbumin and count of lymphocyte were measured on the fourth day before operation, the first day and the eighth day after operation. Re~ suits Times of flatus and defecation of Group B and Group C patients were shorter in group A （P 〈 0. 05）, Group C showed significant decrease in the body weight, serum albumin, serum prealbumin and count of lymphocyte compared with those in others （ P 〈 0. 05 ）. Conclusion Preoperative and postoperative enteral nutrition can significantly promote the rehabilitation of postoperative patients with gastric carcinoma.
