手术联合放疗对食管癌伴单侧锁骨上淋巴结转移治疗的分析

摘　　要：目的探讨食管癌伴单侧锁骨上淋巴结转移的手术联合放射治疗的价值及临床意义。方法回顾分析2000年9月至2009年9月所收治食管癌伴单侧锁骨上淋巴结转移患者42例的临床资料。结果手术切除率95．2％（40／42），无围手术期死亡，并发症发生率14．3％。随访8～118个月，3年生存率，5年生存率分别为41．9％，14．1％。结论食管癌伴单侧锁骨上区淋巴结转移患者只要一般情况好，无其他重要器官转移，均可采用手术切除原发灶及转移灶，术后追加放射治疗的综合治疗，可以改善生活质量，延长生存时间。Objective To explore the value and the clinical significance of the treatment of surgery combined with radiotherapy for esophageal cancer with unilateral supraclavicular lymph node metastasis. Methods The clinical data of 42 patients of esophageal cancer with unilateral supraclavicular lymph node metastasis,27 males and 15 females, aged 54.3 （42 -68 ）, that underwent surgery combined with radiotherapy from September 2000 to September 2009 were analyzed retrospectively. Resuits The resection rate was 95.2%. There were no perioperative death. The Complication rate was 14.3%. With 8 to 118 months following - up, the 3 - year survival and 5 - year survival rates were 41.9% and 14. 1%, respectively. Conclusions The esophageal cancer patients with unilateral supraclavicular lymph node metastasis can underwent surgery combined with radiotherapy,if they have good conditions and no other major organ metastasis. This will improve their quality of life and prolong survival time.
