摘  要:目的探讨初发性翼状胬肉的外观形态和显微切除手术中组织解剖结构的关系。方法 47例(47眼)门诊初发性翼状胬肉患者,术前进行眼前段照相,定量测量胬头部长度、颈部宽度、肥厚程度及半月皱襞移位情况;显微切除手术中测量角膜缘侵犯宽度、角膜前弹力层侵蚀范围、巩膜暴露区大小,分析胬肉组织的解剖结构与胬肉外观形态的关系。结果翼状胬肉对角膜缘的侵蚀宽度约为颈部宽度的3/4,角膜缘侵犯宽度与半月皱襞移位程度相关,角膜前弹力层侵蚀范围和巩膜暴露区大小与胬肉的肥厚程度和半月皱襞移位程度相关。结论胬肉侵及眼表组织的范围较外观形态小,胬肉的结膜组织存在牵拉折叠结构,胬肉的肥厚程度和半月皱襞移位与胬肉对眼表组织侵犯的范围相关。Objective To investigate the morphology feature and anatomic structure of pterygium in microsurgery excision.Methods 47 cases(47 eyes) with primary pterygium pterygium undertaken microsurgery excision from outpatients were enrolled.Standardized anterior segment photography were taken for all the 47 pterygiums.The pterygium head length,neck width were quantitatively measured with computer software(Photoshop 11.0).Pterygium translucency and plica semilunaris displace were clinically graded.The extent of invaded limbus and Bowman's lamina,scleral exposure were measured by gauge in the operation.Results The extent of invaded limbus is as three-quarters smaller than neck width.The extent of invaded limbus and Bowman's lamina,scleral exposure were related to the plica semilunaris displace and Pterygium translucency.Conclusion The extent of the ocular surface that pterygium invaded is smaller than the pterygium contour.Plica semilunaris displace and petrygium translucency is related to the invaded ocular surface extent and conjunctiva defect in microsurgery excision.
