原发性肺非霍奇金淋巴瘤的CT诊断

摘　　要：目的:探讨原发性肺非霍奇金淋巴瘤的CT表现特点。方法:回顾性分析经临床病理证实的13例原发性肺非霍奇金淋巴瘤的CT表现。结果:高度恶性弥漫性大B细胞非霍奇金淋巴瘤4例14个病灶,其中肿块型2例3个肿块,结节型1例3个结节,混合型1例8个病灶;低度恶性小B细胞非霍奇金淋巴瘤9例24个病灶,其中肿块型1例1个肿块,结节型1例2个结节,肺叶实变型3例5个病灶,混合型4例16个病灶。在肿块型3例4个肿块中,支气管充气征3个,伴不规则坏死空洞2个;结节型2例共5个结节,其中结节边缘见磨玻璃征及内见支气管充气征2个;肺叶实变型3例5个病灶,实变区内均见支气管充气征,其中边缘见磨玻璃征2个病灶,支气管腔明显狭窄阻塞1个病灶;混合型5例24个病灶,为多种表现并存,其中支气管充气征10个,磨玻璃征12个。CT增强5例,其中轻度强化3例,中度强化2例。结论:原发性肺非霍奇金淋巴瘤CT表现形式多样性,CT表现结合临床资料有助于该疾病的诊断。Objective：To describe and analyze computed tomography（CT） features of primary pulmonary non-Hodgkin′s lymphoma（PPNHL）.Methods：CT manifestations of 13 cases with PPNHL confirmed by pathology and clinical follow-up were retrospectively analyzed.Results：14 high grade malignant PPNHL foci were found in 4 case.From which,3 lesions of mass type in 2 cases;3 lesions of nodular type in 1 case;8 lesions of mixed type in 1 case.24 low grade PPNHL foci were found in 9 cases.The foci classification was as follows：one lesion of mass type in 1 case;2 lesions of nodular type in 1 case;5 lesions of lobar consolidation type in 3 cases;16 lesions of mixed type in 4 cases.Of the 4 mass-type foci in 3 cases,3 had air bronchogram sign and 2 had irregular necrosis cavities.Of the 5 nodule-type foci in 2 cases,2 had ground-glass opacities and had air bronchogram in nodules.Of the 5 lobar consolidation-type in 3 cases,5 had air bronchogram,and 2 had ground-glass opacities,and 1 had bronchial lumen obstruction.Of the 24 mixed-type foci in 5 cases,10 had air bronchogram and 12 had ground-glass opacities.On enhancement scans,three mild enhancement foci and 2 moderate enhancement foci in 5 cases were found.Conclusion：CT manifestations of PPNHL are varied,but some specific imaging features of PPNH still exist,CT examination combined with clinical manifestations is helpful for accurate diagnosis of PPNHL.
