全膀胱尿道切除回肠膀胱术治疗膀胱癌26例疗效探讨

摘　　要：目的探讨膀胱癌行全膀胱及尿道切除回肠膀胱术(Bricker术)的临床治疗效果。方法回顾性分析26例膀胱癌患者,一期施行膀胱及尿道全切回肠代膀胱尿流改道术的临床资料。结果所有病例术后均恢复良好,出现近期并发症5例,无远期并发症。26例均获随访1～10年,平均6.2年,5年生存率67.5%。结论对于膀胱癌侵犯尿道及前列腺无法保留尿道的患者,采取一期切除膀胱及全尿道回肠膀胱尿流改道术,操作简单易行,术后恢复快、并发症少、疗效确切,推荐为此类患者的优先选择。Objective To investigate the clinical result of the operations on the patients for radical cystectomy and urethra rescetion and revision urinary conduct bladder.Methods Review and analyze the data of 26 patients having urinary cancer at one time,who were operated by cutting off the whole urinary and urethra and using ileum to take the place.Results All the patiets recovered well after the operation.Only 5 of them had recent complication.None had deferend complication.The 26 patients have been followed up by regular visits for 1 to 10 years,6.2 years on average and 67.5% of them survived for 5 years.Conclusions It is an easy and practical treatment for urinary cancer patients who have urinary and prostatitis transfer and cannot keep urinary and urethra to perform the operation of removing urinary and urethra at one time and using ileum to take the place.After the operation,patients will recover fast and have less complication.The treatment has really good curative effect.So we recommend it to patients for priority.
