内镜窄带成像与染色技术在诊断胃部疾病中的比较

摘　　要：目的:探讨窄带成像(NBI)技术与染色技术对胃部疾病的诊断价值。方法:对328例常规内镜检查有病灶者行NBI检查、亚甲蓝染色检查、亚甲蓝醋酸染色检查,观察3组检查方法胃黏膜腺管形态及微血管结构变化,进行病变显示清晰度评分,评价病变性质,并与术后病理检查结果进行比较。结果:NBI组腺管结构清晰度评分优于亚甲蓝染色组(χ2=16.58,P<0.05),但与亚甲蓝醋酸染色组比较,差异无统计学意义(χ2=2.31,P<0.05);微血管结构显示评分NBI组显著高于亚甲蓝染色组及亚甲蓝醋酸染色组(χ2=18.33,P<0.05;χ2=16.74,P<0.05);NBI组镜下诊断与病理诊断符合率(83.8%)显著高于亚甲蓝染色组(73.2%)(χ2=17.12,P<0.05),但与亚甲蓝醋酸染色组(85.4%)比较,差异无统计学意义(χ2=2.74,P>0.05)。结论:NBI诊断胃部疾病效果确切,亚甲蓝醋酸染色诊断效果与NBI相近,值得推广。Objective： To investigate the value of narrow-band imaging（NBI） technology and chromoendoscopy technique in the diagnosis of gastric disease.Methods： 328 cases which had been detected suspicious lesions by conventional endoscopy examination were treated with NBI and chromoendoscopy（the meilan and acetate meilan）.The stomach mucous membrane appearance and changes of microvessel structure of the three groups were observed.The lesion-definitions were scored,lesion-natures were estimated,and they were compared with the pathological findings after operation.Results： The score of tubular structure definition of NBI group was better than meilan group（χ2=16.58,P〈0.05）,but there were no significant difference between NBI group and acetate meilan group（χ2=2.31,P〈0.05）.The score of micrangium structure got by NBI group was significantly higher than the meilan group and acetate meilan group（χ2=18.33,P〈0.05;χ2=16.74,P〈0.05）.Differences in the diagnostic coincidence of NBI group（83.8%） was higher than meilan group（73.2%）（χ2=17.12,P〈0.05）,but there was no significant difference beween NBI group and acetate meilan group（85.4%）（χ2=2.74,P〈0.05）.Conclusion： NBI is helpful and useful for the diagnosis of gastric disease.There are no significant difference between NBI and meilan-acetate.Promotion of the mailan-acetate method is very necessary.
