调和肝脾法对肠易激综合征免疫学作用机制研究

摘　　要：目的:观察调和肝脾法对肠易激综合征患者免疫学指标的作用。方法:临床选择腹泻型肠易激综合征患者为研究对象,治疗前后分别检测患者血中IgG、IgA、IgM、C3、C4;血T淋巴细胞总数与亚群数目;IL-2水平,并设正常对照组。结果:腹泻型肠易激综合征患者治疗前血清IgM含量明显升高,外周血总T淋巴细胞、T8+细胞数目显著降低;淋巴细胞转化功能明显低下,IL-2低于正常组,经调和肝脾法治疗后过高的IgM含量显著下降(P<0.01),淋巴细胞转化功能增强,抑制性T淋巴细胞(T8+)恢复正常,IL-2也接近正常水平。结论:腹泻型肠易激综合征患者体液免疫亢进,细胞免疫功能低下,免疫调节机制紊乱,调和肝脾法中药能抑制某些亢进的体液免疫指标,增强细胞免疫功能,从而起到调节免疫功能的作用。Objective： To observe the immunological mechanism of harmonizing the liver and spleen in patients with irritable bowel syndrome. Methods： The patients with diarrhea-predominant irritable bowel syndrome were choiced for the study in clinic. Before and after treatment, blood IgG, IgA, IgM, C3, cg; blood T lymphocytes and the total number of subgroups; IL -2 （IL -2） Level was checked in patients as well as the normal control group. Results： Before treatment, in patients with the diarrhea-predominant irritable bowel syndrome, serum IgM were significantly elevated, the total T lymphocytes and T8^＋ number of ceils in peripheral blood were a significantly reduced; the transforming function of lymphocyte was significantly lower. IL-2 was lower than that in the normal group. After reconciling the liver and spleen, IgM decreased significantly （P 〈0.01）, lymphocyte function enhanced, inhibit T-lymphoeytes （T8^＋） backed to normal, IL-2 also was closed to normal level. Conclusion： In patients with diarrhea-predominant irritable bowel syndrome, the humoral immunity function is higher, and the cellular immunity function is lower. Some traditional Chinese medicine reconciling the liver and spleen play a role in regulating immune function by inhibiting the hyper humoral immunity and enhancing cellular immunity.
