简易营养评价法及其精法在老年压疮患者营养评价中的应用

摘　　要：目的比较简易营养评价法（Mini-Nutritional Assessment,MNA）和简易营养评价精法（Short-form Mini-NutritionalAssessment,MNA-SF）对老年压疮患者营养状况评价的差异,选择最适合该患者群的临床评价方法。方法对155例老年压疮患者进行MNA和MNA-SF测评,人体测量、生化检测,比较2种评价方法和各项传统营养评价指标对老年压疮患者营养状况的评价结果。结果传统营养评价指标用于老年压疮患者,营养不良检出率一致性较差（24.5%~87.1%）;MNA测评显示营养不良、潜在营养不良和营养正常检出率分别是80.7%、17.4%和1.9%;MNA-SF测评显示营养不良和营养正常检出率分别是97.4%和2.6%;与理想体质量百分数、体质量指数、上臂围、小腿围、血红蛋白、血清白蛋白、血清前蛋白均有显著相关,相关系数在0.255～0.618（P〈0.01）,MNA和MNA-SF评分之间亦有高度相关性,相关系数为0.836（P〈0.01）。结论老年压疮患者营养不良发生率高,与传统营养评价指标相比,MNA和MNA-SF皆是评价老年压疮患者营养状态的更简单有效方法,但在客观性和简易性方面,MNA-SF优于MNA。Objective To compare the effect of Mini-Nutritional Assessment（MNA） and Short-form Mini-Nutritional Assessment（MNA-SF） in evaluating nutritional status of older patients with bedsore,and therefore to choose the most suitable clinical assessment for them.Methods The assessment with MNA and MNA-SF,anthropometric measurement and biological examination were carried out in 155 older patients with bedsore.Results The result of MNA suggested the percentage of malnutrition,potential malnutrition and normal nutrition were 80.7%,17.4% and 1.9%,respectively and MNA-SF indicated that the percentage of malnutrition and normal nutrition were 97.4%,2.6%,respectively,both of which were significantly related with desirable body weight percentage,body mass index,mid-upper arm circumference,calf circumference,hematoglobin,serum albumin and serum preprotein with a correlation coefficient ranging between 0.255 and 0.618（P0.01）.There was also significant relation between MNA and MNA-SF,with a correlation coefficient of 0.836（P0.01）.Conclusion The incidence of malnutrition in older patients with bedsore remains high.Both MNA and MNA-SF are simple and effective in evaluating nutritional status of older patients with bedsore.MNA-SF,however,performs better than MNA for its objectivity and simplicity.
