农村和城区肇事重性精神疾病患者临床特征比较

摘　　要：目的了解农村和城区肇事重性精神疾病患者的临床特征及其差异。方法对我院2010年9—12月收治的176例农村和城区肇事重性精神疾病患者的一般资料及临床资料(包括姓名、性别、年龄、户籍、病程、入院次数、入院原因、精神疾病诊断、简明精神病量表(BPRS)与日常生活能力量表(ADL)评分以及合并躯体疾病等)进行统计分析。结果农村和城区肇事重性精神疾病患者性别、年龄、病程、住院次数及入院原因比较,差异均无统计学意义(P>0.05)。精神疾病诊断构成比比较,差异有统计学意义(P<0.05);入院时BPRS评分比较,差异无统计学意义(P>0.05);ADL评分比较城区患者低于农村患者,差异有统计学意义(P<0.01));入院时合并低钠/低钾比较农村患者比城区患者合并率高,差异有统计学意义(P<0.05));合并贫血、营养不良、外伤、传染病比较,农村与城区患者差异均无统计学意义(P>0.05))。结论农村和城区肇事重性精神疾病患者精神症状均较为严重且多合并躯体疾病,但仍存在着一定的城乡差异,需引起社会的重视。Objective To explore the differences in the clinical features between the rural and urban troublemakers with psychiatric disorders.Methods All the 176 rural and urban troublemakers with psychiatric disorders who were hospitalized from September to December,2010 were involved.The general information and clinical data including name,gender,age,household register,course of disease,times of hospitalization,reasons of hospitalization,diagnoses of psychiatric disorders,scores of BPRS and ADL,and comorbid physical diseases were statistically analyzed.Results There were no differences in the genders,ages,courses of disease,times of hospitalization,reasons of hospitalization（P0.05）,but there was a significant difference in the diagnoses of psychiatric disorders between the rural and urban patients（P0.05）.There were no differences in the BPRS scores between the rural and urban patients（P0.05）,but the ADL scores of the urban patients were significant higher than those of the rural patients（P0.01）.The rural patients had more comorbid hyponatremia / hypokalemia than the urban patients（P0.01）,but there were no differences in comorbid anemia,malnutrition,infectious diseases,traumas between the rural and urban patients（P0.05）.Conclusion Most of the rural and urban troublemaker with psychiatric disorders are in a bad condition with severe psychiatric syndromes and comorbid physical diseases,but there are still some differences between them,all the rural and urban troublemaker with psychiatric disorders should be paid close attention.
