胶质瘤术中荧光素钠实时显像的应用分析

摘　　要：目的探讨荧光素钠术中实时显像在胶质瘤手术中的应用价值。方法回顾性分析128例术中应用荧光素钠实时显像的胶质瘤病人的临床资料,其中肿瘤位于不利手术区域78例,位于有利手术区域50例。根据术中使用荧光素钠后肿瘤是否显色分为染色组和未染色组。结果在有利手术区域,染色组全切除40例(95%),次全切除2例;未染色组全切除2例(25%),次全切除6例,两组全切除率差异有统计学意义(P<0.001)。在不利手术区域,两组全切除率差异无统计学意义(P=0.122),但染色组术后KPS评分明显高于术前(P=0.002),未染色组手术前后KPS评分无显著差异(P=0.335)。染色组平均手术时间、术后平均住院时间均较未染色组缩短(P=0.002和P=0.001)。结论术中荧光素钠实时荧光显像对胶质瘤治疗和预后有意义。Objective To explore the application of intraoperative real-time imaging of sodium fluorescein in glioma.Methods Clinical data of 128 patients with glioma undergoing intraoperative real-time imaging of sodium fluorescein were analyzed retrospectively.The tumors located in the area disadvantageous for surgery in 78 cases,and in the advantageous surgical area in 50.The patients were divided into dyeing group and non-dyeing group according to whether the tumor dyed after using sodium fluorescein.Results In the advantageous surgical area,total tumor removal was achieved in 40 cases（95%） and subtotal removal in 2 in dyeing group,while total removal was achieved in 2 cases（25%） and subtotal removal in 6 in non-dyeing group.The total removal rate was much higher in dyeing group than the non-dyeing group（P0.001）.However,in the disadvantageous area for surgery,there was no significant difference in the total removal rates between the two groups（P=0.122）,but the postoperative KPS scores was higher than the preoperative scores in dyeing group（P=0.002）,while there was no statistical difference in non-dyeing group（P=0.335）.The operative time and postoperative hospital stay of dyeing group were much shorter than those of the non-dyeing group（P=0.002,P=0.001）.Conclusions The intraoperative real-time imaging of sodium fluorescein is a valuable procedure for treatment and prognosis of glioma.
