脂肪肝及其腹围与代谢综合征的相关性研究

摘　　要：目的为进一步探索代谢综合征的临床本质,比较观察对象中非酒精性脂肪肝的有无与腹围的大小中代谢紊乱何者更严重。方法对2007年10～12月我院1416例体检人员的临床资料进行分析,依据腹围的大小与脂肪肝的有无将1416例体检人员分为正常腹围非脂肪肝组、腹型肥胖非脂肪肝组、正常腹围脂肪肝组和腹型肥胖脂肪肝组,观察脂肪肝与代谢综合征的分布情况。结果 "健康"体检人群中脂肪肝及代谢综合征发病率高。腹型肥胖不一定有脂肪肝,其致代谢综合征(MS)的情况不如正常腹围脂肪肝组的严重,有脂肪肝组无论腹围大小,其收缩压(SBP)、舒张压(DBP)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FPG)、等代谢指标紊乱程度均高于非脂肪肝组(P<0.05)。结论脂肪肝与代谢综合征存在明显相关性,要重视脂肪肝的严重性,早期干预,以预防心血管事件及代谢综合征的发生。Objective In order to further exploring clinical essence of metabolic syndrome,and to investigate the correction between fatty liver and abdominal circumference as well as metabolic syndrome （MS）.Methods The clinical data of 1 416 individuals coming to our hospital for physical examination were analyzed.The 1 416 subjects were divided into four groups based on with or without fatty liver and size of abdominal circumference：group A （with normal abdominal circumference and non-fatty liver）,group B （with abdominal obesity and non-fatty liver）,group C （with normal abdominal circumference and fatty liver）,and group D （with abdominal obesity and fatty liver）.Results The incidence of fatty liver and MS were high.The patients with abdominal obesity were not always with fatty liver.In group C and group D,the metabolic disorder degree of systolic blood pressure （SBP）,diastolic blood pressure （DBP）,alanine aminotransferase （ALT）,aspartate aminotransferase （AST）,triglyceride （TG）,total cholesterol （TC）,fasting blood glucose （FPG） of were higher than those of group A and group B （P0.05）.Conclusion There is an obvious correction between fatty liver and MS.Patients with fatty liver should be given early prevention in order to prevent the occurrence of cardiovascular events and metabolic syndrome.
