改良Forsus矫治器与肌激动器治疗安氏Ⅱ类下颌后缩疗效对比研究

摘　　要：目的:比较改良式Forsus矫治器与肌激动器治疗安氏Ⅱ类下颌后缩病例的临床疗效。方法:应用改良式Forsus矫治器(F组)与肌激动器(A组)联合固定矫治器分别对20例安氏Ⅱ类下颌后缩病例进行功能性矫治,治疗前后摄头颅定位侧位片,进行Pancherz头影测量分析。SPSS 12.0统计学软件进行配对t检验。结果:F组治疗时间(20.5±2.1)个月,A组治疗时间(26.6±2.7)个月,差异具有显著性。两组病例治疗后患者下颌后缩面型显著改善。前牙覆盖减小,磨牙关系纠正为Ⅰ类关系。SNB增大,ANB减小,以上测量项目治疗前后比较,均具有显著的统计学差异,但组间差异并不显著。治疗后,平面的变化A组较明显。结论:改良式Forsus矫治器与肌激动器均可以有效促进下颌骨的生长发育,协调上下颌骨的位置关系,显著改善Ⅱ类咬合关系和下颌后缩面型。两者的矫治效率和矫治疗效均无明显差异。肌激动器组的双期矫治在治疗时间上明显长于改良式Forsus组。Objective：To evaluate and analysize the effects between with modified Forsus appliance and activator in the treatment of Class II madibular retrusion by Pancherz.Method：40 children with Class II malocclusion and mandibular retrusion were selected and divided into two groups.20 patients treated with this new appliance and other 20 treated with activator.Cephalometric radiographs were taken and analyzed at the pre-treatment time（T1） and the post-treatment time（T2） with the Pancherz analysis.Students＇ t-test was used to determine if there were significant differences between the 2 time periods.Result：After treatment,profiles of the two groups were obviously improved.The trends were similar in overjet and the molar relationship,SNB and ANB.all had significant differences.Conclusion：Both the modified Forsus appliance and activator appliance combined with straight wire appliance can effectively stimulate the mandibular growth,balance the jaw relationship,and correct the Class II mandibular retrusion.
