肝病与非肝病血液透析患者透析过程中血压变化的临床观察

摘　　要：目的观察肝病维持性血液透析患者透析中血压的变化。方法选取2010-01-01-2010-01-31首都医科大学附属北京佑安医院血液透析中心54例稳定的维持性血液透析患者,分为慢性肝病组、肝硬化组及非肝病组。观察三组患者透析过程中血压的的变化情况。结果透析过程中高血压的发生率分别为慢性肝病组13.58%、肝硬化组13.06%、非肝病组16.13%。三组之间比较无统计学差异。透析过程中低血压的发生率分别为慢性肝病组16.36%、肝硬化组33.16%、非肝病组16.13%。三组之间相互比较有统计学差异。结论肝病及非肝病血液透析患者透析过程中都可出现血压升高,肝硬化较慢性肝病及非肝病患者血液透析过程中更易发生低血压,临床应加强对肝硬化患者透析过程中低血压发生的预防。Objective To observe the incidence of hypotension and hypertension during hemodialysis（HD） in patients without hepatopathy and with chronic hepatopathy. Methods 54 stable maintained HD patients from lto 31January, 2011 were divided into3 groups：chronic hepatitis（CH） group,liver cirrhosis（LC） group and without hepatopathy group. Observe the incidence of hypotenion and hypertension during hemodialysis between the 3 groups. Result Pa- tients of CH group underwent hemodialysis for 324 times, occurred hypertension for 44 times and hypotension for 53 times, the incidence were 13.58 % and 16.36%. Patients of LC group underwent hemodialysis for 199 times, occurred hypertension for 26 times and hypotension for 66 times, the incidence were 13. 060% and 33.16%. Patients of without hepatopathy group underwent hemodialysis for 155 times, occurred hypertension for 25times and hypoten- sion for 25 times, the incidence were 16.13% and 16.13%. There was no statistical significance in the incidence of hypertension during hemodialysis between the 3 groups. There was statistical significance in the incidence of hypo- tension during hemodialysis between the CH group and the LC group： There was statistical significance in the inci- dence of hypotension during hemodialysis between the without hepatopathy group and the LC group： There was no statistical significance in the incidence of hypotension during hemodialysis between the without hepatopathy group and the CH group. Conclusion The incidence of hypotension during hemodialysis in the LC patients is higher than that in the CH patients and without hepatopathy group, The prevention of hypotension during hemodialysis in LC patients should be given much more attention and strengthened.
