书写痉挛患者异常皮质活动的功能磁共振成像研究

摘  要:目的:研究书写痉挛患者皮质异常功能活动,利用功能磁共振成像,探讨其病因。方法:分别对10名书写痉挛患者和10名健康志愿者进行手指写、执笔写任务期间的功能磁共振成像,对数据经过分析处理,获得患者组和正常对照组2个任务的平均脑激活图并进行比较。结果:与正常对照组相比较,患者组初级感觉运动区和辅助运动区表现为与书写任务相关的明显激活(P<0.05),而双侧的后顶叶皮质激活在2个不同任务期间均较正常减弱。结论:书写痉挛患者存在运动和感觉皮质的异常活动,反应了其运动皮质的去抑制和感觉处理过程的障碍,并且感觉处理过程的异常可能在其病因过程中起重要作用。Objective: To detect whether there is abnormal cortical activation in writer' s cramp patients with functional magnetic resonance imaging(fMRI). Methods: Cortical activation during writing in 10 patients with writer's cramp was detected with fMRI,with 10 healthy volunteers as controls. The mean cerebral activation images of the two groups were acquired and compared with each other. Results: Compared with controls, the patient group showed greater activation of the primary sensorimotor cortex and supplementmotor area during writing with finger(P〈0.05).However, there was less activation of the bilateral posterior parietal cortex (PPC)than controls during writing. Conclusions: The results indicate that inhibition in the motor cortices and impaired to information results in abnormalities of activation in motor and cortices in patients with writer' S cramp. Furthermore, an impaired capacity to integrate sensory information might play an important role in thepathophysiology of writer's cramp.
