摘  要:目的:通过回顾性分析甲状腺良恶性结节超声影像特点,探讨甲状腺结节良恶性的超声诊断价值。方法:回顾性分析2006年11月—2010年5月因甲状腺结节就诊并行手术治疗的患者253例,共记录结节418个。术前行甲状腺超声检查并记录结节边界、回声性质、回声结构、钙化情况、结节血流等超声特征。结果:术后病理良性结节291个,恶性结节127个。边界不清楚、实性、低回声结构、钙化、周边血流等超声特征在恶性结节中较为常见,以上述特征为标准判断恶性结节的敏感性分别为:52%(43%~61%),87%(79%~92%),87%(80%~93%),48%(39%~57%)和54%(44%~62%);特异性分别为:69%(63%~74%),74%(69%~79%),46%(40%~52%),74%(69%~7 9%),86%(82%~90%)和48%(42%~54%);诊断的优势比分别为:2.38(1.55~3.65),18.64(10.49~33.10),5.84(3.29~10.35),5.80(3.58~9.39)和1.05(0.69~1.60)。结论:综合分析甲状腺结节超声影像特点可以提高其鉴别良恶性结节的诊断价值。Objective:To analysis the ultrasonic characteristic of malignant nodules and evaluate the diagnostic value of doppler ultrasonography on predicting thyroid malignant nodes.Methods:A total of 253 patients with 418 thyroid nodules were dectcted by doppler ultrasonography in our hospital from November 2006 to May 2010 retrospectively.The ultrasonography features such as shape margin,echotexture,ehogenicity,presence and type of calcification,blood flow inside or around nodules were evaluated before surgery.Results:Of 418 nolules,291 were benign and 127 were malignant.Ill-defined border,solid and hypoechogenicity calcification and blood flow around the node was frequently seen in margilant nolules.According to the above ultrasonography features the sensitivity is 52%(43%~61%),87%(79%~92%),87%(80%~93%),48%(39%~57%) and 54%(44%~62%).The specificity is 69%(63%~74%),74%(69%~79%),46%(40%~52%),74%(69%~79%),86%(82%~90%)and 48%(42%~54%).The diagnosis odds ratio is 2.38(1.55~3.65),18.64(10.49~33.10),5.84(3.29~10.35),5.80(3.58~9.39)and 1.05(0.69~1.60)respectively.Conclusion:A combined use of conventional ultrasonography features can improve the accuracy of differential diagnosis.
