尿脱落细胞LivinαmRNA表达在膀胱癌诊断中的价值

摘　　要：目的评价尿脱落细胞中LivinαmRNA表达对诊断膀胱尿路上皮癌的价值。方法选择43例膀胱尿路上皮癌患者(病例组)和31例非尿路上皮肿瘤患者(对照组),分别进行尿脱落细胞的LivinαmRNA检测(逆转录多聚酶链反应法)和尿细胞学检查。分析尿脱落细胞中LivinαmRNA的表达及其与肿瘤病理分级和分期的关系,比较两种诊断性试验的敏感度、特异度和Youden指数。结果病例组尿脱落细胞中LivinαmRNA的阳性表达率为81.4%(35/43),明显高于对照组的6.5%(2/31)(P<0.05);病例组尿脱落细胞中LivinαmRNA表达与肿瘤的病理分级和分期无关(P>0.05)。尿脱落细胞LivinαmRNA检测的敏感度和Youden指数均显著高于尿细胞学检查(P<0.05),而两者的特异度差异无统计学意义(P>0.05)。结论检测尿脱落细胞LivinαmRNA可作为膀胱尿路上皮癌较理想的无创性诊断方法。Objective To investigate the diagnostic value of Livinα mRNA expression in urinary exfoliative cells of bladder urothelial carcinoma（BUC）.Methods The expression of Livinα mRNA and urinary cytology in urinary exfoliative cells were determined with reverse transcriptase-PCR in 43 patients with BUC（study group） and 31 cases without urothelial tumor（control group）.The relationship of Livinα mRNA expression with the pathological grade and stage of BUC were analyzed.The sensitivity,specificity and Youden index were compared between two kinds of diagnostic trial.Results The positive rate of Livinα mRNA expression in study group was 81.4%,which was higher than 6.5% in control group（P〈0.05）.The expression of Livinα mRNA in urinary exfoliative cells of BUC was not correlated with the pathological grade and stage in study group（P〉0.05）.The sensitivity and Youden index of Livinα mRNA in urinary exfoliative cells of BUC were significantly higher than those in urinary cytology（P〈0.05）,whereas the difference of specificity was not statistically significant between two kinds of diagnostic trial（P〉0.05）.Conclusion The detection of Livinα mRNA in urinary exfoliative cells may be an ideal method for noninvasive diagnosis of BUC.
