创伤合并内毒素诱发弥散性血管内凝血模型的建立

摘　　要：目的:建立一种创伤后感染继发弥散性血管内凝血(DIC)模型,观察其病理变化,为临床防治DIC提供依据。方法:将家兔随机分为正常对照组(C组)、单纯创伤组(T组)、内毒素组(E组)、创伤合并内毒素组(T+E组)。在造模1h和造模后2、6、12h取血,检测PT、APTT、PLT、D-二聚体等指标,观察家兔精神等一般情况变化及取家兔肺、肾脏进行组织病理学观察。结果:T组、E组、T+E组PLT、APTT、PT、D-二聚体在不同时相,与C组比较差异有统计学意义(P<0.05或P<0.01)。与T组相比,T+E组6h时PT、APTT、D-二聚体均明显延长或升高,PLT明显降低,差异有统计学意义(P<0.05或P<0.01)。与E组相比,T+E组12hPT、APTT明显延长,差异有统计学意义(P<0.05或P<0.01);PLT与D-二聚体从6h起差异有统计学意义(P<0.05或P<0.01)。T组、E组、T+E组肺肾病理均可见微血栓形成,T+E组尤甚。结论:创伤联合内毒素可制备DIC兔模型,反映创伤后继发感染致DIC发生的病理过程,并且该模型各项指标较单纯创伤或感染引起的DIC更有意义。Objective To set up an experimental rabbit model of disseminated intravascular coagulation （DIC） induced by trauma and endotoxin,and to observe its pathological changes.Methods Twenty-four rabbits were divided randomly into 4 groups：control group （Group C）,trauma group （Group T）,endotoxin group （Group E）,trauma and endotoxin group （Group T ＋ E）.Prothrombin time （PT）,activated partial thromboplastin time （APTT）,plantlet （PLT）,and D-dimer were tested,the general states of rabbits were observed,and then the tissues of the lung and kidney of the rabbits were taken for histopathological observation.Results There were obvious differences of PT,APTT,PLT,and D-dimer between Group C and the other three groups （P 0.05 or P 0.01）.PT,APTT,and D-dimer were obviously prolonged or elevated,and PLT was significantly decreased at 6h in Group T ＋ E as compared to those in Group T （P 0.05 or P 0.01）.PT and APTT in Group T ＋ E were prolonged at 12 h,and PLT and D-dimer were significantly changed at 6h as compared to those in Group E （P 0.05 or P 0.01）.Micro-thrombus formation in lung and kidney were observed in Group T,E,and was especially serious in Group T ＋ E.Conclusion The experimental rabbit model of DIC induced by trauma and endotoxin could be set up,which was much better than that of DIC induced by trauma or infection alone.
