摘  要:目的:研究原发性血小板增多症血栓形成与JAK2-V617F突变及各临床相关因素的关系。方法:纳入原发性血小板增多症患者64例,继发性血小板增多症患者50例,分析其血栓发生率和JAK2-V617F突变率及相关临床因素的关系。结果:原发性血小板增多症患者血栓发生率高于继发性血小板增多症(P<0.01)。原发性血小板增多症血栓组JAK2-V617F突变率高于非血栓组(P<0.05);血栓组白细胞数、vWF:Ag、年龄高于非血栓组(P<0.05),AT-Ⅲ、PC活性低于非血栓组(P<0.05);血栓组血红蛋白量、血小板数、PS值与非血栓组无明显差异(P>0.05)。结论:原发性血小板增多症患者血栓形成率高,与JAK2-V617F突变、高龄、高白细胞、高vWF:Ag及低AT-Ⅲ、低PC相关。Objective To study the relationship between JAK2-V617F mutation,clinical parameters and thrombosis in patients with essential thrombocythemia (ET).Methods Sixty-four patients with ET (ET group),50 patients with secondary thrombocythemia (ST group) were included in this study.Correlation between the incidence of thrombosis and JAK2-V617F mutation rate,clinical parameters were analyzed.Results The incidence of thrombosis in ET group was higher than that in ST group(P 0.01).JAK2-V617F mutation rate was higher in patients with thrombosis than that in patients without thrombosis in ET group (P 0.05).The white blood cell count,vWF:Ag,and age in patients with thrombosis were significantly higher than those in patients without thrombosis in ET group(P 0.05),while antithrombin Ⅲ (AT-Ⅲ) level,protein C activity were lower (P 0.05).The hemoglobin level,platelet count,protein S (PS) level were not significantly different between the two groups (P 0.05).Conclusions The incidence of thrombosis in patients with ET was high,and was correlated with JAK2-V617F mutation,old age,high white blood cell count,high vWF:Ag levels,and low AT-Ⅲ and PC levels.
