胆囊癌肉瘤的CT影像学分析

摘　　要：目的探讨胆囊癌肉瘤(carcinosacoma of gallbladder)的CT影像表现特征。方法回顾性分析经手术病理证实的5例胆囊癌肉瘤的CT影像表现,5例均行CT平扫,3例增强。结果 5例均为单发;3例位于胆囊颈部,1例位于胆囊底部,1例位于胆囊体部;均见胆囊内有结节状或菜花状软组织影突入,密度不均匀,3例可见坏死,1例见出血;伴胆囊体积增大、基底部胆囊壁增厚;4例伴有胆囊内结石;4例可见肝脏等周围组织浸润及肿大淋巴结;3例可见肝内转移及远处转移灶;2例出现腹腔积液;1例可见肝内外胆管扩张;增强的3例中,增厚的胆囊壁、腔内软组织肿块有轻中度不均匀强化。结论全面综合地分析胆囊癌肉瘤的CT影像学特征对胆囊癌肉瘤的诊断及其鉴别具有一定的价值。Objective To investigate the CT images performance characteristics of gallbladder sarcoma.Methods The CT images performanced for five gallbladder sarcomas confirmed by surgery and pathology were respectively analyzed,in which 5 cases were through CT scan and 3 were through enhanced scan.Results 5 cases were all single in which,3 were in gallbladder neck,1 was in the bottom and 1 was in the body.Nodular or cauliflower-like soft tissue penetrated in the gallbladder of all the patients with uneven density in which there were 3 cases of necrosis and 1 case of bleeding.The volume of gallbladder was increased,and the base of the gallbladder wall was thicken.The stones in gallblaabber were found in 4 patients.There were 4 cases of tissue invasion in the liver and other tissues with lymph nodes.Liver metastasis and distant metastasis were found in 3 cases.And there were 2 cases of ascites and 1 case of extrahepatic bile duct dilatation in the liver.The gallbladder wall was thicken and intraluminal soft tissue mass was mildly to moderately enhanced heterogeneous in 3 cases.Conclusion Comprehensive analysis to CT imaging features of gallbladder sarcoma had a certain value for diagnosis and differential of gallbladder sarcoma.
