摘 要:目的探讨磁刺激疗法治疗坐骨神经损伤的临床效果及机制。方法将60只SD大鼠随机分为观察组、对照组各24只和假手术组12只,前两组制备坐骨神经损伤模型。制模后观察组给予表面场强为0.09 T磁刺激,30 min/d1,次/d;其余两组不予干预。各组分别于第2、48、1、2周采用免疫组化法检测L4~L5脊髓运动神经元内生长相关蛋白(GAP-43)表达,于12周末行电生理检查测定神经传导速度(MCV)。结果与对照组比较,观察组各时点GAP-43表达明显升高(P均<0.05),术后第12周再生神经传导速度加快、波幅升高、潜伏时缩短(P<0.05)。结论磁刺激疗法能促进大鼠周围神经损伤后MCV的恢复,其机制可能为增加损伤脊髓运动神经元中GAP-43的表达。Objective To investigate the effect of magnetic stimulation treatment to rats sufferring from sciatic nerve crush injury on motor conduction velocity(MCV) and the expression of growth associated protein( GAP-43 ) of the corre- sponding motor neurons. Methods Selected 60 SD rats, then divided into experimental group(24 rats), control group(24 rats) and sham operation group( 12 rats) randomly. Crushed sciatic nerve to make models of peripheral nerve injury in the first two groups. The experimental group had magnetic stimulation on the wounds of sciatic nerve after crush. The control group had invalid stimulation after crush. The sham operation group only exposed the unilateral sciatic nerve. The sciatic nerve of the three groups were observed by means of electrophysiological at 2,4,8,12 weeks after opertation to study the effect of magnetic stimulation treatment on the expression of GAP-43 of L4 - L5 motor neurons, and immunohistochemical staining was used at 12 weeks after opertation to study the MCV. Results The expression of GAP-43 in L4 - L5 motor neurons of the first two groups was higher than that of the control group at the same time point after operation ( P 〈 0.05 ). The regenerated nerve of the experimental group had faster conduction velocity, higher amplitude and shorter latency time than the control group 12 months after the operation ( P 〈 0.05 ). Conclusion The magnetic stimulation can promote the recovery of MCV, the mechanism may up-regulate the GAP43 expression of the motor neurons in the damage spinal cord.
