头颈部神经鞘瘤31例临床分析

摘　　要：目的探讨头颈部神经鞘瘤的治疗方法。方法回顾性分析2001年1月～2009年12月收治的31例头颈部神经鞘瘤的临床资料。全部患者均行手术治疗,按是否完整切除肿瘤包膜将其分为完整切除肿瘤包膜组(18例)及部分切除肿瘤包膜组(13例),比较2组的肿瘤复发率、源神经损伤率。结果术后28例患者获随访6个月～8年不等,3例失访(其中完整切除肿瘤包膜组1例,部分切除肿瘤包膜组2例),2组均无复发。术后完整切除肿瘤包膜组并发源神经损伤5例,部分切除肿瘤包膜组无源神经损伤,完整切除肿瘤包膜组源神经损伤率高于部分切除肿瘤包膜组(P<0.05)。结论手术彻底切除肿瘤是治疗头颈部神经鞘瘤最有效的方法。神经鞘瘤包膜是否完整切除,均不影响其预后。在不损伤神经纤维束的前提下,可将神经鞘瘤包膜一并切除。Objective To investigate the treatment of neurinoma of the head and neck.Methods From January 2001 to Decrmber 2009,the clinical data of 31 cases with neurinoma of the head and neck were retrospectively reviewed and analyzed.Results All patients underwent surgery,28 patients were followed up after 6 months to 8 years,3 cases were lost（including 1case of complete resection of the tumor capsule group and 2 cases of partial resection）,2 groups had no recurrence.After complete resection of the tumor capsule set of concurrent source of nerve injury in 5 cases,partial removal of the tumor capsule group passive nerve injury,complicated by the source between the 2 groups showed no significant difference in nerve injury（P0.05）.Conclusion Tumor resection is still the most effective treatment available for patients with neurilemmomas of the head and neck.Resection of tumor should be performed as soon as possible with careful protection of the nerve function.
