摘　　要：目的探讨不同类型淋巴瘤患者血浆中肿瘤坏死因子-α(TNF-α)表达水平及其与不同临床特征和预后的关系。方法采用酶联免疫吸附(ELISA)法测定132例恶性淋巴瘤患者(病例组)及132例健康对照者(对照组)血浆中TNF-α的水平,分析TNF-α表达水平与IPI评分、有无全身症状和Ann Arbor分期的相关性。结果 (1)病例组TNF-α在霍奇金淋巴瘤(HL)、非霍奇金淋巴瘤(NHL)和弥漫大B细胞淋巴瘤(DLBCL)、小细胞淋巴瘤(SLL)表达水平高于对照组,差异无统计学意义(P>0.05)。在浆细胞瘤(PCN)和淋巴母细胞淋巴瘤(LBL)中TNF-α水平低于对照组,在LBL中TNF-α水平与对照组比较差异有统计学意义(P<0.05)。在HL和NHL血浆中TNF-α表达水平差异无统计学意义(P>0.05),而LBL与PCN、DLBCL、SLL组间差异有统计学意义(P<0.05)。(2)高危组淋巴瘤患者血浆TNF-α表达水平明显高于对照组和低危组,差异有统计学意义(P<0.05)。有全身症状组淋巴瘤患者血浆TNF-α表达水平明显高于对照组和无全身症状组,差异有统计学意义(P<0.05)。Ann ArborⅣ期淋巴瘤患者血浆TNF-α表达水平明显高于对照组及Ⅰ～Ⅲ期,差异有统计学意义(P<0.05)。结论 TNF-α的表达水平与IPI评分、有无全身症状和Ann Arbor分期有关,可作为淋巴瘤患者判断预后的辅助指标。Objective To explore the expression of plasma tumor necrosis factor-α（TNF-α） and the association with different clinical features and prognosis in different subtypes of lymphoma.MethodsEnzyme-linked immunosorbent assay（ELISA） method was used to detected levels of plasma TNF-α in 132 cases of malignant lymphoma and 132 healthy controls.Results（1）The levels of plasma TNF-α in HL,NHL and DLBCL,CLL were higher than control groups,but the difference was not statistically significant（P0.05）.LBL and PCN the level of TNF-α were lower than the control groups,there was statistically significant（P0.05） in LBL.Between HL and NHL,there was no significant difference（P0.05）.There were statistically significant in LBL with PCN,DLBCL,SLL.（2） Plasma TNF-α expression level in high risk of lymphoma patients was significantly higher than control group and low risk group（P0.05）.Plasma levels of TNF-α in high risk group of lymphoma was significantly higher with control group and low risk group（P0.05）.Expression levels of plasma TNF-α in Ann Arbor Ⅳ was significantly higher than Ann Arbor Ⅰ～Ⅲ groups（P0.05）.ConclusionPlasma level of TNF-α was associated with high risk,clinical symptoms and Ann Arbor stages,it could be used as a marker to determine prognosis of patients with lymphoma.
