体重指数、生活满意度和负性生活事件在功能性消化不良发病中的作用

摘　　要：背景:社会和心理因素与功能性胃肠病密切相关,两者在功能性消化不良(FD)中的作用逐渐成为研究热点。目的:探讨体重指数(BMI)、生活满意度和负性生活事件在FD发病中的作用。方法:纳入2009年6月～2010年1月上海市第一人民医院的80例FD患者和80名健康对照者,采用调查问卷和生活事件量表(LES)评估各种因素在FD发病中的作用。结果:FD组的BMI和生活满意度显著低于对照组(P<0.05),而性别、年龄、婚姻状况、文化程度、职业情况、居住地、抽烟/饮酒、体育锻炼、医疗费用支付方式与对照组相比无明显差异。FD组负性生活事件频数和刺激量明显高于对照组(P<0.05),而正性生活事件无明显差异;Logistic回归分析发现FD的发生与负性生活事件相关。结论:FD的发生与BMI和生活满意度可能相关,负性生活事件可能是FD的危险因素。Background：It has been found that social and psychological factors are important in the pathogenesis of functional gastrointestinal disorders,and the roles of these factors in functional dyspepsia（FD） have become a hotspot of study.Aims： To investigate the roles of body mass index（BMI）,degree of life satisfaction and negative life events in the pathogenesis of FD.Methods：Eighty FD patients and 80 healthy controls from Jun.2009 to Jan.2010 at Shanghai First People＇s Hospital were enrolled.Questionnaire and life event scale（LES） were adopted to investigate the effects of various factors in the pathogenesis of FD.Results：BMI and degree of life satisfaction in FD group were significantly lower than those in controls（P0.05）,and no significant differences in gender,age,marital status,level of education,profession,residential place,smoking or drinking,physical exercise and mode of medical payment were found between FD groups and controls. The frequency and magnitude of negative life events in FD group were significantly higher than those in controls（P0.05）, while no significant difference in positive life events was found between the two groups.Logistic regression analysis showed that FD was relevant with negative life events.Conclusions：The pathogenesis of FD may be associated with BMI and degree of life satisfaction,and negative life events may be a risk factor of FD.
