产后抑郁与T细胞免疫调节的相关性研究

摘　　要：目的探讨产后抑郁对产妇细胞免疫功能的影响及其临床意义。方法采用产后抑郁量表对346例产妇进行调查,评价产后心理状态,按有无产后抑郁分为产后抑郁组及对照组,采用流式细胞仪检测两组产妇血清T淋巴细胞分类情况,比较两组产妇CD3+、CD4+、CD8+、CD4+/CD8+表达水平。采用直线回归分析EPDS评分与CD3+、CD4+、CD8+、CD4+/CD8+表达水平之间的相关性。结果抑郁组产妇CD3+、CD4+表达水平低于对照组,CD8+表达比例高于对照组,CD4+/CD8+表达水平低于对照组,相关性分析结果显示:抑郁组产妇EPDS评分与CD3+、CD4+、CD4+/CD8+表达水平呈现负相关,与CD8+表达水平呈正相关。结论产后抑郁能够引起产妇产后T细胞免疫功能下调,影响机体健康,应采取必要的心理干预措施,促进产妇产后身心健康。Objective To investigate the correlation between postpartum depression and T cell-mediated immunity and analized the clinical significance. Methods All 346 cases of parturient were invested with EPDS and were divided into postpartum depression groups and control group accroding to mental state, T cell subset were detected with flow cytometry, expressive level of CD3＋, CD4＋, CD8＋and CD4＋/CD8＋ were contrasted between postpartum depression groups and control group, correlation score of EPDS with level of CD3＋, CD4＋, CD8＋ and CD4＋/CD8＋ were analized with linear regression in postpartum depression groups. Results Expressive level of CD3＋ and CD4＋ were lower in postpartum depression groups than in control groups, expressive level of CD8＋ was higher in postpartum depression groups than in control groups, expressive level of CD4＋/CD8＋were lower in postpartum depression groups than in control groups, correlation test shown there was negative correlation between score of EPDS and expressive level of CD3~, CD4＋, CD4＋/CD8＋, there was postive correlation between score of EPDS and expressive level of CD8＋. Conclusion Postpartum depression could coused decreasing of T cell-mediated immunity in parturient, mental intervention should were taken and improved physical and mental health of parturient.
