摘 要:目的 观察促血管生成素Ang-1、Ang-2及其受体Tie-2在鼻咽血管纤维瘤(JNA)中的表达情况,探讨其与鼻咽血管纤维瘤扩展性生长的关系.方法 采用免疫组化法检测并比较Ang-1、Ang-2、Tie-2蛋白及CD34在30例JNA(JNA组)、20例鼻咽部慢性炎症组织(对照组)中的表达情况.结果 JNA组Ang-1、Ang-2及Tie-2阳性表达率分别为80%(24/30)、76.67%(23/30)、100%(30/30),对照组分别为80%(16/20)、30%(6/20)、45%(9/20),两组患者Ang-1阳性表达率差异无统计学意义(P >0.05),JNA组Ang-2及Tie-2阳性表达率均显著高于对照组(均P<0.01).对照组CD34呈低阳性表达、JNA组呈强阳性表达,微血管密度(MVD)分别为17.6007±3.03881、14.0267±2.93158,差异有统计学意义(P<0.01).Ang-2在JNA组不同年龄、性别间的表达差异无统计学意义(P >0.05),而在Ⅲ+Ⅳ期的阳性表达率(90%)明显高于Ⅰ+Ⅱ期(50%),差异有统计学意义(P<0.05).肿瘤周边组织中Ang-1的表达与Ang-2的表达呈负相关(P<0.05),Ang-2蛋白的表达与MVD呈正相关(P<0.05).结论 Ang-1与Ang-2在JNA外周组织血管生成中相互拮抗,Ang-2可能决定JNA外周组织血管生成和肿瘤扩展性生长.当Ang-2高表达而Ang-1低表达时,促进JNA外周组织的血管生成;反之则抑制JNA外周组织血管生成.推测Angs及其受体系统在JNA外周组织血管生成和肿瘤扩展性生长中的调节作用是以Ang-2的作用为主导.Objective To investigate the expression of angiopoietin-1(Ang-1), angiopoietin-2 (Ang-2) and their acceptor Tie-2 proteins in juvenile nasopharyngeal angiofibroma (JNA) and its clinical significance. Methods Ang-1,Ang-2 and Tie-2 proteins and CD34 were measured by immunohistochemistry in 30 cases of JNA and 20 cased of chronic inflammation of pharynx nasalis. Results The positive rates of Ang-2 and Tie-2 proteins in JNA were 76.7% and 100.0% respectively, which were significantly higher than those in chronic inflammation of pharynx nasalis; while the positive rates of Ang-1 both in JNA and chronic in- flammation were 80.0%. The expression rate of CD34 in JNA was lower than that in chronic inflammation(P 〈0.05 ).The expression of Ang-2 protein was correlated with clinical staging of JNA (P 〈0.05), but not with gender and age of patients (P 〉0.05). In the peripheral tissue of JNA the expression of Ang-2 was negatively correlated with Ang-1 protein (r=-0.4654,P 〈0.05), while Ang-2 protein was positively correlated with MVD (r=0.5083,P 〈0.05). No correlation of Ang-1 protein and Ang-2/Ang-1 ratio with MVD was observed. Conclusion The results indicate that Ang-2 antagonizes Ang-1 in the angiogenesis of peripheral tissue of JNA; Angs and their receptor regulate the angiogenesis and Ang-2 may play a dominant role in the vessel growth.
