摘　　要：目的 观察UroplakinⅢ（UPⅢ）在侵袭性膀胱癌中的表达,及其与血管内皮生长因子（VEGF）表达的相关性.方法 选取74例根治性膀胱切除术标本,同期选取10例正常膀胱组织作为阳性对照,用免疫组化法检测UPⅢ和VEGF的表达情况,并分析UPⅢ与VEGF的表达与临床病理学指标的关系,及两者之间的相关性.结果 UPⅢ在侵袭性膀胱癌中的表达（33.8%）与在正常膀胱组织中的表达差异有统计学意义（P＜0.01）,其表达缺失与膀胱癌高分级、肌层浸润、淋巴管浸润呈正相关（P＜0.05）;VEGF在正常膀胱移行上皮中均为阴性表达,在侵袭性膀胱癌组织中阳性表达53例,表达率为71.6%,明显高于正常膀胱组织（P＜0.05）,其表达与分级、临床分期及是否伴淋巴结转移密切相关（P＜0.05）,而与患者年龄、性别、是否有淋巴管浸润无关.UPⅢ和VEGF的表达存在明显负相关（P＜0.05）.结论 UPⅢ的表达缺失和VEGF的过度表达参与了侵袭性膀胱癌的发展,可作为判断侵袭性膀胱癌预后的分子指标.Objective To study the expression of Uroptakin III and VEGF in aggressive bladder cancer. Methods Immunohistochemical staining for UP III and VEGF was carried out in 74 archival radical cystectomy and 10 normal specimens .The expression of UP III and VEGF were compared clinical pathologic parameters and the relationship between them. Results UP Ul espression was significantly decreased in bladder cancer tissues（33.8%） compared with normal controls（P〈0.01）. Loss of UP III expression was associated with high-grade ,muscle-invasive cancer,lymphovascular invasion （P〈0.05）. Normal transition al epithelial cell were tested negative for VEGF.Among the bladder transitional cell carcinoma there were 24 cases tested positive for VEGF, the rate of expression was 71.6%,and it was much more apparent than for those of normal bladder tissues （P〈0.05）.The expression of VEGF is associated with high-grade ,muscle-invasive cancer, lymph node metastasis （P〈0.05）,but not associated with patient＇s age, gender, CIS, lymphovascular invasion.The expression UP III is correlated with the expression of VEGF（P〈0.05） Conclusion Loss of UP III expression and Up-regulation VEGF expression may be involved in the progression of aggressive bladder cancer. UP III and VEGF can be used as a useful molecularmarker for prognosis of aggressive bladder cancer.
