微型接骨板治疗髁突矢状骨折23例疗效观察

摘　　要：目的总结微型接骨板应用于髁突矢状骨折(sagittal fracture of the m and ibu larcondyle,SFMC)的手术治疗经验。方法 对术前及术后3个月、术后6个月患者的咬合关系、最大开口度、张口偏斜、髁突术后形态等进行观察比较。结果①术后3个月平均开口度(33.40mm)及单侧病例平均张口偏斜(1.43mm)均较术前明显改善,术后6个月(39.16mm,1.24mm)与术后3个月相比较仍有改善(P<0.05)。②术后3个月(平均17.21mm)与术前(平均12.41mm)比较髁突高度有显著差异,术后6个月(平均17.15mm)与术后3个月无显著差异。③单侧髁突骨折12例,与对侧比较,术前髁突高度有显著差异(P<0.05),术后3个月(P=0.776)与术后6个月(P=0.238)均无统计学意义(P>0.05)。④术后髁突未见明显吸收,微型钛板无明显变形。结论微型接骨板治疗SFMC可以取得较好的效果,开口度较术前明显改善,咬合关系恢复良好,术后髁突无明显吸收。Objective To summarize the experiences in the treatment of sagittal fracture of the mandibular condyle（SFMC） with miniature bone plate.Methods 23 patients with sagittal fracture of the mandibular condyle（among these patients,6 cases with unilateral bone fracture） were fixed with miniature bone plate.Occluding relation,maximum opening,opening deflexion and the height of the mandibular condyle were measured from the clinic and radiology before the treatment and at the postoperative 3 or 6 months respectively.These data were analyzed statistically.Results ①The average opening（33.40mm） and opening deflexion inunilateral cases（1.43mm） on 3 months after the surgery has been significantly improved compared to that before the surgery.In addition,the indications after the surgery have been improved further on 6 months（39.16mm and 1.24mm） than that on the 3 months（P0.05）.②There was significant difference in the height of mandibular condyle between postoperation 3 months（average 17.21mm） and the preoperation（average 12.41mm）.However,there was no significant difference between 6 months（average 17.15mm） and 3 months after the operation.③Analyzing the change of height of the mandibular condyle in unilateral cases,comparing to contralateral,there was significant difference before treatment The data,however,at both 3 and 6 months after operation has no obvious difference in statistic（P0.05）.④Bone absorption and miniature titanium plate deformation were not found during the follow-up.Conclusion SFMC fixed with miniature bone plate was an effective treatment.Using this method,opening,occluding relation were all improved and there was no absorption on the mandibular condyle.
