妊娠合并急性胰腺炎大鼠胎盘组织细胞黏附分子E-选择素的表达及意义

摘　　要：目的:探讨细胞黏附分子E-选择素(E-selectin)在妊娠合并急性胰腺炎大鼠血清与胎盘组织中的表达水平及其与胎盘功能的关系。方法:妊娠7 d大鼠胆胰管逆行注射5%牛磺胆酸钠,制作妊娠合并急性胰腺炎大鼠模型。分为正常妊娠组、妊娠合并急性胰腺炎1 h、4 h、8 h、12 h、24 h组(每组10例)。酶显色法测定血清脂肪酶水平,免疫分析法测定血清E-selectin浓度,逆转录聚合酶链反应技术及免疫印迹法检测胎盘组织E-se-lectin的mRNA及蛋白表达。E-selectin水平与胰腺损伤评分、胎盘功能评分进行相关性分析。结果:正常妊娠大鼠血清与胎盘组织内E-selectin呈低水平表达。妊娠合并胰腺炎12 h与24 h组血清E-selectin水平显著升高(P<0.05),胎盘组织E-selectin的mRNA及蛋白表达明显增加(P<0.05);E-selectin的升高与胰腺损伤程度(r=0.908)、胎盘功能损伤程度(r=0.803)呈正相关。结论:血清E-selectin的升高与胎盘组织损害相关;E-selectin可能参与了妊娠合并急性胰腺炎胎盘损伤的病理生理过程。AIM： To explore the role of E-selectin in the pathogenesis of acute pancreatitis in pregnant rats.METHODS： Pancreatitis was induced by retrograde injection of 5% sodium taurocholate through biliopancreatic duct.The animals were sacrificed in normal pregnancy group,and 1 h,4 h,8 h,12 h,24 h after induction of pancreatitis.The severity of pancreatitis was evaluated by detecting serum lipase and calculating pancreatic pathological score.Placenta injury was evaluated by pathological observation and serum E-selectin detection.The mRNA and protein expression of E-selectin in placenta tissues was determined by RT-PCR and Western blotting,respectively.RESULTS： The expression of E-selectin in serum and placenta was at low levels in normal pregnancy group.The expression of E-selectin in serum and placenta tissues markedly increased 1 h after induction of pancreatitis in pregnant animals,and gradually reached its peak at 12 h and 24 h（P0.05）.Linear regression analysis showed that the concentrations of E-selectin was significantly related to the degree of pancreatic injury（r=0.908） and placenta injury（r=0.803）.CONCLUSION： The concentration of E-selectin is significantly related to the degree of placenta injury.E-selectin may be involved in the pathogenesis of leukocyte adhesion in placenta of pregnant rats with acute pancreatitis.
