摘 要:目的探讨肾脏继发性淋巴瘤的CT表现,以提高对该病的认识和诊断能力。方法回顾性分析7例肾脏继发性淋巴瘤患者的资料,均为非霍奇金淋巴瘤。所有病例均行腹部CT平扫+增强扫描。结果 7例肾脏继发性淋巴瘤,5例为结节(肿块)型,其中多发2例,单发3例,邻近病灶侵犯型1例,肾周淋巴瘤1例,病灶直径从数毫米至7.2 cm。CT平扫所有病变密度均匀,其中4例为等密度,3例为稍高密度,平扫CT值30.6~38.3 HU(平均34.6 HU);增强动脉期CT值42.7~65.7 HU(平均49.7 HU),静脉期CT值50.5~86.5 HU(平均64.1 HU),平衡期CT值55.3~115.2 HU(平均71.3 HU),3例中度均匀延迟强化,4例明显均匀延迟强化。结论肾脏继发性淋巴瘤的CT表现形式多样,密度均匀,增强扫描呈进行性延迟强化,平衡期扫描有助于提高小病灶的发现率,明确诊断需要结合临床资料。Objective To investigate the CT characteristics of renal secondary lymphoma,and to improve the understanding and diagnosis of this disease.Methods Retrospective analysis was performed in 7 patients with non-Hodgkin lymphoma accompanied by renal secondary lymphoma.The patients were examined by abdominal plain and contrast-enhanced CT.Results In the 7 patients with renal secondary lymphoma,5 patients were diagnosed as nodular(mass) type including 2 multiple types and 3 solitary types;one patient was diagnosed as a type of direct extension from the adjacent lesion,and 1 was a perinephric lymphoma type.The diameters of the renal lesions ranged from a few millimeters to 7.2 cm.The plain CT showed that the density of all lesions were homogeneous,including 4 isodense and 3 slightly hyperdense,and the values were 30.6 to 38.3 HU(with a mean value of 34.6 HU).The CT values in the enhanced arterial phase,venous phase and equilibrium phase were 42.7 to 65.7 HU(with a mean value of 49.7 HU),50.5 to 86.5 HU(with a mean value of 64.1 HU) and 55.3 to 115.2 HU(with a mean value of 71.3 HU),respectively.The contrast-enhanced CT showed 3 moderate and 4 overt homogeneously delayed enhancements.Conclusion The CT characteristics of the renal secondary lymphoma are diverse,the density is homogeneous,and a progressively delayed enhancement is observed by the contrast-enhanced CT.CT scan in the equilibrium phase can elevate the detection rate of small lesions,which combined with the clinical information can help us make a definite diagnosis.
