伴银屑病的急性髓系白血病类型及染色体核型分析

摘　　要：目的:探讨急性髓系白血病(AML)与银屑病之间的关系,并分析伴有银屑病AML患者的染色体核型及疗效特点。方法:调查本院2004～2010年100例AML患者银屑病的发生率,并应用G显带技术分析AML患者染色体核型。结果:AML患者中银屑病伴发率为8%(8/100),其中7例为AML-M3,1例为AML-M5。AML-M3患者的银屑病伴发率为25.9%(7/27),显著高于非M3患者伴发率1.4%(1/73)和总体伴发率8%(均P<0.05)。AML-M5患者银屑病伴发率为5.3%(1/19),其它AML亚型未发现伴有银屑病。73例获得染色体核型患者中,非重现性结构异常发生率13.7%(10/73)。伴银屑病AML患者非重现性结构异常发生率为50%(3/6),伴实体瘤AML患者发生率为100%(2/2),均高于不伴银屑病或其它实体瘤的AML患者发生率7.7%(5/65)(P<0.05)。AML-M3中2例伴t(15;17)平衡易位之外结构异常患者均达到完全缓解。结论:伴银屑病的AML患者中以AML-M3较为多见,伴银屑病AML患者染色体核型容易出现非重现性结构异常,伴银屑病AML-M3中染色体非重现性结构异常可能对疗效无影响。Objective：To explore the relationship between acute myeloid leukemia（AML）and psoriasis and analyze the characteristics of chromosome and efficacy in AML patients with psoriasis.Methods：The incidence of psoriasis in 100 AML patients who treated in the hospital from 2004 to 2010 were investigated,and G banding techniques was used to analysis karyotype.Results：The incidence of psoriasis in AML patients was 8%,and seven patients were AML-M3,one patient was AML-M5.The incidence of psoriasis in AML-M3 patients was 25.9%（7/27）,which was higher than the incidence of non-M3 patients 1.4%（1/73）（P0.05）and AML patients 8%（P0.05）.The incidence of psoriasis in AML-M5 patients was 5.3%（1/19）,and the psoriasis was not seen in the other subgroups.The incidence of nonrecurrent chromosome aberration was 13.7%（10/73）in all the 73 patients who acquired chromosome results.The incidence of nonrecurrent chromosome aberration was 50%（3/6）in the AML patients with psoriasis,and the incidence was 100%（2/2）in patients with tumor,and both of which were higher than patients without psoriasis or tumor.Two AML-M3 patients with nonrecurrent aberrations besides t（15;17）translocation achieved complete remission.Conclusion：AML-M3 subgroup is common in AML patients with psoriasis,and nonrecurrent chromosome aberrations are more common.Nonrecurrent chromosome aberrations maybe have no effect on efficacy of AML-M3 patients with psoriasis.
