摘 要:放疗引发二重癌,已成为威胁癌症患者生存质量的重要因素。本文对临床放疗引发的二重癌进行风险预测分析,并对基于二重癌风险预测的计划评估方式展开了探索性研究。选择临床胰腺癌病例,使用FDS团队自主研发的精确放射治疗软件系统(Accurate/Advanced Radiotherapy System,ARTS)设计两个调强放疗计划,并对正常组织各重要器官的二重癌风险进行了预测,并以此对两种计划进行了评估。结果表明,该病人在接受放射治疗后,最有可能发生二重癌的是肾癌和胆囊癌(Plan 01:肾5.35%,胆囊3.10%;Plan 02:肾4.52%,胆囊4.57%);二重癌分析结果表明:在满足临床剂量分布要求的前提下,Plan 02更为合理。本文建立了一种通过临床放疗计划系统预测二重癌风险的方法,并通过比较二重癌风险进行了临床放疗计划的评估分析。Estimates of second primary cancers risk after radiotherapy has become increasingly important for comparative treatment planning. A new method based on the treatment planning system to estimate the risk of second primary cancers was introduced in this paper. Using the Advanced/Accurate Radiotherapy Treatment System (ARTS), a treatment planning system developed by the FDS team, the risk of second primary cancer was estimated over two treatment plans for a patient with pancreatic cancer. Based on the second primary cancer risk, the two plans were compared, It was found that, kidney and gall-bladder had higher risk to develop second primary cancer. A better plan was chosen by the analysis of second primary cancer risk. The results showed that this risk estimation method we developed could be used to evaluate treatment plans.
