经内镜植入粒子化疗对食管癌的治疗研究

摘　　要：目的探讨经内镜粒子植入化疗对食管癌姑息治疗的临床价值。方法回顾性对照分析54例行食管癌姑息治疗患者的临床资料,研究组:内镜IC化疗33例;对照组:支架植入21例。结果①IC作用缓慢,于12周达最大效果(52.4%),24周与支架植入差别无统计学意义;②IC患者临床近期(12周内)随肿瘤分化程度不同疗效有统计学差异(P<0.05);而远期(24周)无统计学差异;支架植入组随分化程度不同远期(24周)效果统计学差异明显;③IC组并发胸痛、纵隔脓肿、食管气管瘘明显低于支架植入组(P<0.05)。结论①IC近期治疗效果可能不如支架植入,远期治疗效果无差别;②肿瘤分化程度低对IC近期疗效较好,IC及支架植入对高分化患者远期治疗效果均优于低分化者;③IC患者出现严重并发症明显较支架植入组低。Objective To investigate the therapeutic effect of implantation chemotherapy for esophageal carcinoma.Methods 33 patients with esophageal carcinoma given implantation chemotherapy（IC group） and 21 cases with stents implantation therapy（SI group） were analyzed retrospectively.Results ①IC group reached maximum effect at the end of 12 weeks（52.4%）,and there were no difference between IC groups and SI group at 24 weeks.②Statistical difference between the two groups in short term was significant（P0.05）and showed related to cell differentiation except at 24 weeks.③To IC group,severe complication incidence was less such as chest pain,esophageal-airway fistula,bleeding,mediastinal abscess.Conclusion ①IC was less effective than SI in short term on esophageal carcinoma.②Low grade tissue differentiation may have better response to IC.③There were less complication for IC on esophageal carcinoma.
