颈动脉内中膜厚度的超声定量分析和颈动脉粥样硬化危险因素的研究

摘　　要：目的探讨动脉健康评估(AHP)技术定量分析颈动脉内-中膜厚度(CIMT)的价值,以及影响CIMT的相关因素和颈动脉粥样硬化的危险因素。方法采用AHP技术对890名受检者(其中正常组472人,增厚组197人,斑块组221人)的CIMT进行自动测量,获得颈动脉内-中膜厚度平均值(CIMT-Mean)和颈动脉内-中膜厚度最大值(CIMT-Max),并与受检者的临床资料进行对比分析。结果 CIMT异常组的CIMT-Mean、CIMT-Max、年龄、收缩压和低密度脂蛋白(LDL-C)均高于正常组(P<0.01)。CIMT分别与年龄和LDL-C呈正相关(P=0.000)。年龄增加(OR=1.886,P=0.000)和吸烟(OR=1.656,P=0.000)是颈动脉内-中膜增厚的危险因素;年龄增加(OR=1.914,P=0.000)、LDL-C升高(OR=2.470,P=0.000)和吸烟(OR=1.833,P=0.000)是颈动脉斑块的危险因素。结论超声AHP技术可为定量评价颈动脉粥样硬化程度提供一种方便、准确的测量CIMT的方法,为临床提供客观参考。Objective To explore the value of arterial health package（AHP） for quantitative analysis of carotid intima-media thickness（CIMT） by ultrasonography and affect the relatice factors of CIMT as well as the risk factors of carotid atherosclerosis.Methods Use AHP to inspect automatically the CIMT of 890 persons（among of them,472 in normal group,197 in thickening group and 221 in plaque group）.The mean value of CIMT（CIMT-Mean） and the maximum value of CIMT（CIMT-Max） of the above three groups were obtained and compared with their clinical information of inspected persons.Results CIMT-Mean,CIMT-Max,age,systolic pressure and low density lipoprotein（LDL-C） of the abnormal groups were higher than those of the normal group（P〈0.01）.CIMT was positively correlated with age and LDL-C respectively（P=0.000）.Increasing age（OR=1.886,P=0.000） and smoking（OR=1.656,P=0.000） were risk factors of thickened carotid intima-media.Increased age（OR=1.914,P=0.000）,LDL been high（OR=2.470,P=0.000） and smoking（OR=1.833,P=0.000） were risk factors of carotid plaque.Conclusion AHP can provide a convenient and accurate CIMT measurement for quantitative assessment method of carotid atherosclerosis,and supply objective reference for clinic.
