Hunt-Hess分级Ⅲ-V级颅内前循环动脉瘤的早中期手术体会

摘　　要：目的总结Hunt&Hess分级Ⅲ-V级颅内前循环动脉瘤患者早中期显微手术夹闭的疗效,并探讨其积极手术的可行性。方法回顾分析46例前循环动脉瘤破裂出血后急诊入院,且手术前Hunt&Hess分级Ⅲ--V级的患者,所有患者均接受动脉瘤手术夹闭,脑压高者同时去骨瓣减压(0～12d),以改良Rankin量表评分对患者预后进行评分。结果随访出院后3月,35例患者(76%)预后良好(mRS 0～3分)并返院行颅骨修补术,7例重度残疾,4例死亡。结论 Hunt&Hess分级Ⅲ～V级的颅内前循环动脉瘤患者,积极手术夹闭动脉瘤并去骨瓣减压可获得良好的预后。Objective Summary Hess Hunt Ⅲ-V intracranial aneurysm patients before circulation early or middle micro-surgery GaBi efficacy,and discuss the feasibility of the operation.Methods Review 46 cases of circulation aneurysms before the hemorrhage,and hospital emergency surgery Hunt Hess grading Ⅲ-V patients,all of the patients accept GaBi brain aneurysm surgery,most high pressure to bone disc reduced pressure（0 to 12 d）,with improved outcomes in patients with Rankin rating scale for rating.Results Follow-up after discharge march,35 patients（76%） with good prognosis（mRS0-3 points） and return courtyard travel repair technique,the skull 7 cases of severe disability,4 cases of death.Conclusions Hess Hunt Ⅲ-V intracranial aneurysm patients before surgery,positive cycle GaBi aneurysm and bone disc reduced pressure to receive a good prognosis.
