三维适形放疗联合热疗治疗中晚期肝癌的临床疗效

摘　　要：目的:探讨三维适形放疗联合热疗治疗中晚期肝癌的疗效。方法:经配对后的60例肝癌分为放疗联合热疗、单纯放疗二组,每组各30例。放疗单次剂量180～200cGy,1次/d,5次/周,总剂量为4 000～7 000cGy。热疗采用聚焦超声或体外射频热疗,1h/次,隔日1次。联合治疗组为先行放疗,30min内接受热疗。结果:联合治疗组的总有效率(CR+PR)为93.33%(28/30),显著高于单纯放疗组的70.00%(21/30,P<0.05)。联合治疗组的1年生存率为50.00%(15/30),略高于单纯放疗组的36.67%(11/30)(P>0.05)。除局部皮肤轻度烫伤外,其他副反应联合治疗组较单纯放疗组低。结论:放疗联合热疗治疗中晚期肝癌,能明显的提高临床疗效。Objective：To study the clinical effect of combination of three-dimensional conformal radiotherapy（3-DCRT）and hyperthermia in the treatment of patients with moderate or advanced liver cancer.Methods：60 patients with liver cancer were divided balancedly into 2 arms each with 30 cases：3-DCRT with hyperthermia group or 3-DCRT group.The single dose of the 3-DCRT was 180~200cGy,once a day for 5 times a week.The total dose for patients was 4 000~7 000cGy.Hyperthermia was performed by focused ultrasound or extracorporeal radio-frequency hyperthermia,one hr a time for every other day,or 3 times a week.The therapies for the conbined group were 3-DCRT followd by hyperthermia within 30mins.Results：The total effective rate（CR＋PR）was 93.33%（28/30）for the conbined group,and 70.00%（21/30）for the 3-DCRT only（P0.05）.The one-year survival of the conbined group was 50.00%（15/30）,and 3-DCRT,36.67%（11/30）（P0.05）.The side responses were slightly lower in the conbined group except for the skin thermal slight-damage.Conclusion：The conbination of 3-DCRT with hyperthermia for the treatment of moderate and advanced liver cancer could improve the clinical effectiveness.
