慢性牙周炎患者血清C反应蛋白和龈沟液白细胞介素-6的检测及意义

摘　　要：目的探讨C反应蛋白(C reactive protein,CRF)和白细胞介素-6(interleukin-6,IL-6)在慢性牙周炎患者中的表达,并分析其与慢性牙周炎不同病变程度的相关性。方法收集90例慢性牙周炎患者的临床资料,根据牙周袋探诊深度(pocket depth,PD)和附着丧失(clinical attachment loss,CAL)将其分为轻度、中度和重度,并与同期健康者50例比较。血清CRP测定采用免疫比浊法,龈沟液(gingival crevlcularfluid,GCF)中IL-6检测采用双抗夹心酶联免疫吸附法。结果慢性牙周炎组的血清CRP和GCFIL-6明显高于对照组,差异有统计学意义(P<0.05)。轻度、中度和重度慢性牙周炎患者血清CRF和GCF IL-6显著高于对照组,差异有统计学意义(P<0.05)。血清CRP和GCFIL-6在中度和重度组的表达高于轻度,重度组高于中度组,差异均有统计学意义(P<0.05)。Spearman相关分析显示血清CRP与PD、CAL呈正相关(r=0.309,0.532,P<0.05);GCFIL-6与PD、CAL呈正相关(r=0.337,0.593,P<0.05)。结论慢性牙周炎患者血清CRP和GCFIL-6呈高表达,并与慢性牙周炎病变程度相关性。Objective To explore the expression of serum C - reaction protein （CRP） and interleukin -6 （IL -6 ） levels in the gingival crevicular fluid （GCF） in patients with chronic periodontitis, and to analyze the relationships between these parameters and severity of chronic periodontitis. Methods The clinical data of 90 cases with chronic periodontitis were reviewed. These patients were divided into mild, moderate and severe groups according to pocket depth （PD） and clinical attachment loss （CAL）, and were compared with 1296 healthy people who were selected as control group. Serum CRP and GCF IL- 6 were tested by immune turbidimetry and enzyme- linked immunosorbent assay. Results Serum CRP and GCF IL - 6 were higher in experimental group than control group （P 〈 0.05 ）, higher in moderate and severe groups than mild group （ P 〈 0.05 ）, higher in severe group than moderate group （P 〈 0.05 ）. Serum CRP and GCF IL- 6 related positively to PD and CAL （r =0. 309, 0. 532,P 〈 0.05 ;r = 0. 337,0. 593, P 〈 0.05 ）. Conclusion Serum CRP and GCF IL - 6 are highly expressed in patients with chronic periodontitis, and relate to severity of chronic periodontitis.
