不同手术方式对肾癌患者外周血IL-6和Cor的影响

摘　　要：为了探讨开放手术与后腹腔镜下肾癌根治术对患者白细胞介素-6(IL-6)和血清皮质醇(Cor)的影响,比较机体对两种不同手术方式创伤应激反应,选择行后腹腔镜肾癌根治术患者24例(A组)和开放手术患者26例(B组),分别于术前、术后1、3和5d抽取静脉血测定IL-6和Cor含量进行对比研究。术前A、B两组患者所有指标差异无统计学意义,P>0.05。术后1d,B组患者IL-6、Cor水平均明显高于A组,P<0.05。术后3和5dB组患者IL-6和Cor水平虽高于A组,但两组相比差异无统计学意义,P>0.05。初步研究结果提示,后腹腔镜与开放肾癌根治术在术后d1对机体造成的创伤应激反应差异有统计学意义;术后d3和d5两种手术方式对机体造成的创伤应激反应持续存在,但两者相比差异无统计学意义。The objective of this study was to investigate the influence of retroperitoneoscopic radical nephrectomy（group A） and open nephrectomy（group B） to the level of IL-6 and Cor.Group A had 24 patients and the group B had 26 patients.The IL-6,serum level of corticosteroid were evaluated 1 d,3 d,5 d before and after operation.There was no significant difference between group A and group B than before operation（P0.05）.One day after operation,IL-6 and serum level of corticosteroid increased.There was significant difference between group A and group B（P0.05）.Three and five days after operation,IL-6 and serum level of corticosteroid of group B were higher than that of group A.But there were no significant difference（P0.05）.In conclusion,there is significant difference between retroperitoneoscopic radical nephrectomy and open nephrectomy on the stress response of patients one day after operation.Three and five days after operation,there were no significant difference between group A and group B.
