摘　　要：目的:了解阴道封闭术对老年盆腔器官脱垂(POP)患者相关症状和生活质量的影响。方法:在2005年10月—2010年2月期间,采用盆底困扰量表简表(PFDI-20)及生活质量问卷盆底影响问卷简表(PFIQ-7),对解放军总医院第一附属医院妇产科60例因POP-QⅢ～Ⅳ期重度POP实施阴道封闭手术患者,分别于术前、术后2个月、6个月及1年对患者进行POP相关症状以及生活质量问卷调查。问卷评分越高对生活质量影响越大,反之则对生活质量影响越小。结果:术后2个月、6个月及1年时分别有56人、53人和52人完成问卷调查,随访率分别为93%(56/60例)、88%(53/60例)和87%(52/60例)。PFDI-20与PFIQ-7在术前及术后2个月、6个月及1年时的平均得分依次为(76.78±40.71)分、(15.73±10.55)分、(11.11±8.23)分、(11.06±8.20)分和(82.41±32.49)分、(17.88±18.12)分、(12.93±14.74)分、(12.77±14.64)分,术后评分较术前均有显著下降。结论:阴道封闭手术可以有效缓解老年POP患者的阴道或盆腔不适症状及排尿困扰症状,明显改善患者生活质量。Objective： To observe the effects of colpocleisis on the symptoms and quality of life in elderly women with pelvic organ prolapse （POP）. Methods： From October 2005 to February 2010,60 patients with severe POP stage Ill and IV as classified with POP-Q system underwent total and partial colpocleisis. Patients completed the pelvic floor distress inventory short form （PFDI-20） and pelvic floor impact questionnaire short form （PFIQ-7） before the operation, and they were repeated 2 months, 6 months, and 1 year, respectively, after the operation. Results：Among the 60 patients, 56（93%）, 53（88%） and 52（87%） patients filled the questionnaires 2 months, 6 months, and 1 year after the operation respectively. Mean PFDI-20 score was improved significantly from 76.78± 40. 71 at baseline to 15.73±10. 55 at 2 months, 11.11±8. 23 at 6 months and 11.06±8. 20 at 1 year after the operation. Mean PFIQ-7 score was improved significantly from 82. 41 ±32.49 at baseline to 17. 88 ± 18.12 at 2 months, 12.93±14.74 at 6 months, and 12.77±14. 64 at 1 year after the operation（P= 0. 000）. Conclusions： Colpoeleisis can effectively resolve bothersome vaginal or pelvic and urinary symptoms among elderly women with severe POP and can significantly improve their quality of life.
