丙氨酰-谷氨酰胺联合柴芍承气汤治疗重症急性胰腺炎的临床观察

摘　　要：目的探讨丙氨酰-谷氨酰胺联合柴芍承气汤治疗重症急性胰腺炎的临床疗效。方法将72例重症急性胰腺炎患者随机分为对照组和治疗组,各36例。对照组采用常规治疗,治疗组在常规治疗的基础上加用丙氨酰-谷氨酰胺和柴芍承气汤,两组均监测各项生化指标,治疗结束后进行对比并做统计学分析。结果治疗后治疗组患者血清C反应蛋白水平较对照组显著降低,白细胞计数恢复正常时间、血淀粉酶恢复正常时间、腹痛缓解时间、腹腔积液消失时间、住院时间均较对照组显著缩短,差异有统计学意义(P<0.05);但两组患者的并发症发生率及病死率比较,差异无统计学意义(P>0.05)。结论对于重症急性胰腺炎患者,在常规治疗的基础上尽早使用丙氨酰-谷氨酰胺与柴芍承气汤,可以降低炎症因子C反应蛋白水平,缩短病程,尽早改善症状。Objective To investigate the therapeutic effects of alanyl-glutamine combined with chaishao chengqi decoction in treating severe acute pancreatitis（SAP）. Methods Seventy-two patients with SAP were divided into two groups randomly：the control group（group A,n=36） and the alanyl-glutamine plus chaishao chengqi decoction treatment group（group B,n=36）.All patients of the two groups received the same routine therapy while group B received additional treatment of alanyl-glutamine plus chaishao chengqi decoction.Biochemical indices were monitored in both groups and compared after treatment on statistical basis. Results After treatment,CRP levels were lower while recovery time of white blood cells,blood amylase,abdominal pain,ascites,and hospital stay were shorter than in group B with statistical differences（P0.05）.Incidence of complications and mortality rate were not statistically different（P0.05）. Conclusion In addition to routine treatment,combined alanyl-glutamine and chaishao chengqi decoction for treating SAP results in lower CRP levels and shorter disease courses.
