注射术配合增液承气汤加味口服治疗直肠前突52例

摘　　要：目的：寻找一种简便的、更为有效的治疗直肠前突的方法。方法：将52例经排粪造影等相关检查确诊的直肠前突患者随机分成治疗组和对照组,对照组26例采用传统BLOCK术式（经直肠闭式修补术）,治疗组26例采用直肠黏膜下硬化剂注射术配合增液承气汤加味口服治疗。对两组患者术后治愈率、复发率及术后并发症进行比较分析。结果：治疗组痊愈16例（61.5%）,有效8例（30.8%）,无效2例（7.7%）,总有效率为92.3%,随访6个月~2年,复发1例（3.8%）;对照组痊愈9例（34.6%）,有效10例（38.5%）,无效7例（26.9%）,总有效率为73.1%,随访6个月~2年,复发3例（11.5%）。治疗组术后疼痛持续时间平均为（1.70±0.55）d,对照组平均为（3.30±0.42）d;治疗组术后小便困难7例（26.9%）,对照组术后小便困难17例（65.4%）。两组在治愈率、复发率、疼痛持续时间、小便情况等方面比较,差异均有高度统计学意义（均P〈0.01）。结论：直肠黏膜下硬化剂注射术配合增液承气汤加味口服治疗直肠前突疗效可靠,操作方便,便于临床推广应用。Objective：To Find a simple,more effective methods to treat rectocele.Methods：52 patients with rectocele related by defecography were diagnosed and randomly divided into treatment group and control group,26 cases in control group were treated with traditional surgical BLOCK（by rectum closed repair）,26 patients in treatment group were given sclerotherapy under rectal mucosa combining with Increasing Fluid and Moving Qi Decoction by oral treatment.The cure rate,relapse rate and postoperative complications of two groups after surgery were compared.Results：Treatment group cured were 16 cases（61.5%）,the pain relief patients were 8 cases（30.8%）,no effect patients were 2 cases（7.7%）,total effective rate was 92.3%,followed up for 6 months to 2 years recurrence in 1 case（3.8%）;the control group recovered were 9 cases（34.6%）,the pain relief patients were 10 cases（38.5%）,no effect patients were 7 cases（26.9%）,total effective rate was 73.1%,3 cases of recurrence（11.5%）;The average duration of pain,treatment group had（1.70±0.55） d,control group had（3.30±0.42） d;7 cases in the treatment group（26.9%） had difficulty in urination,in the control group were 17 cases（65.4%）;there were significant differences in cure rate,relapse rate,duration of pain,urine and other aspects of two groups（P〈0.01）.Conclusion：Sclerotherapy under rectal mucosa combining with Increasing Fluid and Moving Qi Decoction by oral treatment is reliable,easy to be operated,and easy to be used in clinical.
