P53表达与食管癌术后切缘复发的关系

摘　　要：目的通过观察P53在食管鳞癌术后切缘及癌组织的表达情况,探讨该基因与食管癌术后切缘复发的关系。方法应用免疫组化染色方法对对照组与复发组切缘及癌灶进行该基因表达水平的检测,并分别对检测结果进行对比研究。同时对复发组与对照组主体癌的临床病理学参数进行对比分析。结果复发组切缘P53表达阳性率(33%)与对照组切缘(10%)间差异有统计学意义(P<0.05)。复发组主体癌P53表达阳性率(70%)与对照组主体癌(43%)间差异有统计学意义(P<0.05)。食管癌的复发与肿瘤浸润深度无显著相关性(P>0.05),而与肿块大小、分化程度及淋巴结转移均有显著相关性(P<0.05)。结论食管癌术后切缘复发可能与p53基因突变有关。切缘及癌灶P53高表达可能是食管癌术后切缘复发的分子基础。Objective By observing the expression of P53 in resection edge and cancer tissue after esophageal squamous cell carcinoma operation, to explore the relationship between expression of P53 and resected edge recurrence in esophageal carcinoma. Methods The immunohistochemical SP staining method of expression of P53 genes was respectively performed on the resected edge of specimens and the main body lump in both contrary group and relapse group. Then test results were compared. While the clinical and pathological parameters of the main recurrent cancer group and control group were analyzed. Results The positive expression rate of P53 protein on the resected edge （33%） in specimens of relapse group was significantly higher than that （10%） of comparative group （P 〈 0.05）. The positive expression rate of P53 protein （70%） on the main body lumps of relapse group was significantly higher than that （43%） of comparative group （P 〈 0.05）. Relapse of esophageal carcinomas had no significant relationship with the infiltration depth of the tumors （P 〉 0.05）, But it was obviously related to the size, histological type and lymphatic metastasis （P 〈 0.05）. Conclusion The gene mutation of P53 are likely to be closely interrelated with the resected edge relapse of esophageal carcinomas. The higher P53 expression in the resected edge and the main body lump is possible molecular basis of resected edge relapse in esophageal carcinoma after operation.
