摘 要:目的通过动态检测脑外伤患者的血清星形胶质源性蛋白(S100β)和神经元特异性烯醇化酶(NSE)的浓度变化,以探讨其在预后判断中的价值。方法选择2008年1月~2011年6月本院诊治的脑外伤患者66例,根据格拉斯哥昏迷计分(Glasgow Coma Scale,GCS)将其分为重伤组和非重伤组,ELISA法动态检测其血清S100β和NSE水平变化。另选取40例年龄、性别与体重指数(BMI)相匹配的健康体检者作为对照。结果两组患者发病后第1天、第3天、第7天血清S100β和NSE含量均增高,且于3d达峰值水平;重伤组和非重伤组第1天、第3天、第7天血清S100β和NSE均显著高于正常对照组(均P<0.01),重伤组亦明显高于非重伤组(P<0.01);Pearson相关分析显示重伤组和非重伤组患者血清S100β均和NSE呈正相关(P<0.05)。结论脑外伤患者血清S100β和NSE水平变化与病情和预后密切相关,联合检测S100β和NSE可以作为评判急性脑外伤患者预后的一项参考指标。Objective To explore the prognostic value of S100β and NSE in patients with traumatic brain injury through dynamically assaying their serum concentration. Methods All 66 patients with traumatic brain injury were selected from January 2008 to July 2011. They were divided into injured group and non-injured group. The serum concentration of S100β and NSE was dynamically assayed. In addition, 40 healthy age, gender, body mass index ( BMI ) matched healthy subjects ( control group ) were set up. Results The serum concentration of S100β and NSE on the 1d,3d,7d in injured group and non-injured group significantly increased. They got to the peak on the third day, then decreased gradually.The serum concentration of S100β and NSE in the two groups were higher than those in control group ( P 〈 0.01 ), and the serum concentration of S100β and NSE in injured group were also higher than those in control group (P 〈 0.01 ). Pearson correlation analysis showed that the serum concentration of S100β in patients on the ld,3d and 7d were positively concerned with NSE (P 〈 0.05 ). Conclusion The serum concentration of S100β and NSE in patients with traumatic brain injury is associated with pathogenetic condition and outcome. Assaying the serum concentration of S100β combined NSE should be of importance as referred indexes.
