胃癌彩色多普勒血流分布特征与VEGF表达

摘　　要：目的探讨胃癌彩色多普勒血流分布特征和病理血管内皮生长因子(VEGF)表达的临床病理关系。方法术前测定各期胃癌患者肿瘤区域的彩色多普勒血流(CDFI),并行半定量分级,对比分析超声测得的胃癌肿块大小、浸润深度、淋巴结转移等不同相关因素中血流分布特征,术后采用免疫组化技术检测相对应标本内VEGF的表达。结果 CDFI血流分级程度与肿瘤大小(P<0.05)、浸润深度(P=0.000)和淋巴结转移(P=0.000)相关;癌组织高于癌旁正常组织(P=0.000),进展期胃癌CDFI血流分级高于早期胃癌(P<0.05);随着血流分级的增高,病理VEGF表达呈上升趋势(P=0.000),两者存在线性相关关系(P<0.05)。结论胃癌彩色多普勒血流分级与肿块大小、浸润深度、淋巴结转移密切相关,与VEGF表达亦有较好的相关性;术前进行胃癌组织CDFI血流检测,可间接评价肿瘤组织内的血管生成状态,为胃癌的预后评估提供有益的信息。Objective To explore the correlation between the feature of color Doppler flow imaging distribution and expression of vascular endothelial growth factor（VEGF） in patients with gastric carcinoma.Methods All patients were received CDFI for measuring the blood grades semiquantitatively during preoperation.Comparative analysis of different factors,such as ultrasound measured gastric tumor size,depth of invasion and lymph node metastasis related to blood flow distribution.VEGF of corresponding samples were immunohistologically stained and measured after operations.Results Blood grades were closely related to the tumor size（P0.05）,the conditions of lymph nodes metastasis（P=0.000）,vascular invasion（P=0.000） and invading depth of tumor tissues（P=0.000）.And value in gastric carcinoma tissues was significantly higher both in carcinoma tissues and advanced than that in normal ones（P=0.000） and early stage（P0.05）.VEGF demonstrated ascending tendency with the higher blood grades of gastric carcinoma tissues（P=0.000） and there was a linear correlation between them（P0.05）.Conclusion Blood grades are closely related to the size,the conditions of lymph nodes metastasis,invading depth of tumor tissues and expression of VEGF.Combination of color Doppler flow imaging and immunohistochemical parameters is useful for evaluating the angiogenesis of gastric carcinoma from different point of view.
