LASEK和LASIK术后对比敏感度的临床研究

摘　　要：目的探讨准分子激光角膜上皮下磨镶术(LASEK)与准分子激光原位角膜磨镶术(LASIK)两种手术方式对近视矫治术后对比敏感度的影响。方法选取条件相匹配的近视患者各24例(48眼),分别采用LASEK及LASIK矫治近视。用对比敏感度测试卡(美国F.A.C.T)检查并对比分析患者在术前和术后1月、3月、6月、1年时的对比敏感度值。结果与术前比较,LASEK组在术后1月对比敏感度值在各空间频率段均明显下降(P均<0.05),术后3月均恢复至术前水平(P均>0.05);LASIK组在术后1月、3月对比敏感度值在各空间频率段均明显下降(P均<0.05),在术后6月恢复至术前水平(P均>0.05)。2组术前各空间频率段对比敏感度值比较差异无统计学意义(P均>0.05);LASEK组在术后1月、3月、6月、1年各空间频率段的对比敏感度值均高于LASIK组(P均<0.05)。结论 LASEK对术后对比敏感度的影响较LASIK小,视觉质量优于LASIK。Objective To evaluate the changes of contrast sensitivity after laser epithelial keratomileusis（LASEK） and laser in situ keratomileusis（LASIK） for correcting myopia.Methods Forty-eight basic data-matched myopic patients treated by LASEK（24 cases,48 eyes） or LASIK（24 cases,48 eyes） for correcting myopia from January to December 2007 were enrolled in this prospective study.Contrast sensitivity of all patients were measured with F.A.C.T（Stereo optical） chart before and 1,3,6 months and 1 year after LASEK or LASIK,and the results were contrasted.Results Compared with preoperative levels,contrast sensitivity of every space frequency at 1 month after LASEK were lower（all P0.05） and recovered in 3 months after surgery（all P0.05）.Compared with preoperative levels,contrast sensitivity of every space frequency in LASIK at 1 month and 3 months after surgery were lower than those of the preoperative levels（all P0.05） and recovered in 6 months later（all P0.05）.Contrast sensitivity of every space frequency had no significantly differences before surgery in the two groups（all P0.05）.However,contrast sensitivity of every space frequency in LASEK were higher than those in LASIK at 1,3,6 months and 1 year after surgery（all P0.05）.Conclusions LASEK has a lower impact on the contrast sensitivity compared with LASIK and better visual quality over LASIK in correcting myopia.
