宫颈病变高危型HPV-DNA的检测价值——附福州市1250例分析

摘　　要：目的 探讨高危型HPV-DNA检测在宫颈病变诊断中的分析价值.方法 对1 250例宫颈病变患者进行HPV-DNA的第2代杂交捕获和液基细胞学的检测,并对结果进行分析.结果 ①随着宫颈液基细胞学检查级别升高,高危型HPV-DNA阳性率增加,HPV感染发生率增加（χ2=155.875,P〈0.05）;②高危HPV-DNA阳性组宫颈高度病变（宫颈上皮内瘤变Ⅱ及以上）发病率明显高于阴性组（χ2=126.680,P〈0.05）.结论 高危型HPV-DNA检测在宫颈病变诊断中有重大意义,高危HPV-DNA阳性组宫颈（宫颈上皮内瘤变Ⅱ及以上）发病率高,宫颈高度病变（宫颈上皮内瘤变Ⅱ及以上）程度也高,高危型HPV检测、宫颈液基细胞学检查及阴道镜下病理活检可提高宫颈高度病变的检出率.Objective To investigate the significance of high risk type human papillomavimses （HPV）-DNA testing in cervical lesions diagnosis. Methods 1 250 cases with cervical lesions were detected with HPV-DNA testing by hybrid capture 2 （ HC- 11 ） and liquid-based cytology test, and the results were analyzed. Results With the increasing of liquid-based cytological examination grade, the positive rate of high risk type HPV-DNA and the incidence rate of HPV infection increased （χ2 = 155. 875, P 〈 0.05 ）. The incidence of high-grade cervical lesions （CIN Ⅱ and above） in positive high risk type HPV-DNA group was significantly higher than that in negative group （χ2 = 126. 680,P 〈 0.05）. Conclusion High risk type HPV-DNA testing is of great significance in diagnosis of cervical lesions. The incidence and the grade of cervical lesions （CIN Ⅱ and above） are high in patients with positive high risk type HPV-DNA. High risk type HPV-DNA testing combined with cervical liquid-based cytological examination and vaginoscopic biopsy can enhance the detection rate of high-grade cervical lesions.
