乳酸脱氢酶β_2微球蛋白及C反应蛋白在急性髓细胞白血病患者血清及脑脊液中检测的临床意义探讨

摘　　要：目的:为了探讨急性髓细胞白血病(AML)患者在初发、缓解、复发及并发中枢神经系统白血病(CNSL)时血清和脑脊液乳酸脱氢酶(LDH)、β2微球蛋白(β2-MG)及C反应蛋白(CRP)水平及临床价值。方法:检测了118例AML患者血清和脑脊液LDH、β2-MG及CRP水平并进行比较,分析其疗效及预后的关系。结果:AML患者的血清LDH、β2-MG及CRP水平在初发、复发组及CNSL组高于对照组(P<0.01);AML患者的血清LDH、β2-MG及CRP水平在初发、复发组高于CNSL组(P<0.01);CNSL组血清LDH、β2-MG及CRP水平高于缓解组,分别为(363±26.38)u/L vs(147±106.21)u/L,(4.75 1.26)mg/L vs(2.67±1.23)mg/L,(17.69±2.96)mg/L vs(6.12±1.13)mg/L(P<0.05)。AML患者的脑脊液LDH、β2-MG及CRP水平在CNSL组高于初发、复发、缓解组(P<0.01);初发、复发组脑脊液LDH、β2-MG及CRP水平高于缓解组,分别为(190±101.91)u/L vs(13291.12)u/L,(284±91.33)u/L vs(132±91.12)u/L(P<0.05);(1.19±1.47)mg/L vs(0.722.10)mg/L,(1.59±1.71)mg/L vs(0.72±2.10)mg/L(P<0.05);(5.52±2.12)mg/L vs(2.353.31)mg/L,(6.90±3.29)mg/L vs(2.35 3.31)mg/L(P<0.05)。结论:AML患者血清和脑脊液中LDH、β2-MG及CRP水平随病情活动而变化与瘤细胞计数、髓外组织侵润相关,可反映肿瘤负荷,对判断AML的病情、预后、复发有帮助,可以作为早期诊断CNSL的有效指标,血和脑脊液LDH、β2-MG及CRP联合检测对于疗效、预后、复发及并发CNSL意义更明显。Objective： To research the level and clinical value of lactate dehydrogenase（LDH）,β2-micro globulin（β2-MG） and C-reactive protein（CRP） of serum and cerebrospinal fluid of acute myelogenous leukemia（AML） patients in their new-onset,remission,relapse and concurrent central nervous system leukemia（CNSL）. Method： We tested the levels of LDH,β2-MG and CRP of serum and cerebrospinal fluid in 118 cases of AML patients,made comparison and analyzed the relation between their curative effect and prognosis. Result： The levels of LDH,β2-MG and CRP of AML patients＇ serum in their new-onset group,relapse group and CNSL group were higher than those in the control group（P〈0.01）.The levels of LDH,β2-MG and CRP of AML patients＇ serum in new-onset group,relapse group were higher than those in CNSL group（P〈0.01）.The levels of LDH,β2-MG and CRP of CNSL group＇s serum were higher than those in remission group,（363±26.38） u/L vs（147±106.21） u/L,（4.75±1.26） mg/L vs（2.67±1.23） mg/L,（17.69±2.96） mg/L vs（6.12±1.13） mg/L respectively（P〈0.05）.The levels of LDH,β2-MG and CRP of AML patients＇ cerebrospinal fluid in CNSL group were higher than those in new-onset group,relapse group and remission group（P〈0.01）;The levels of LDH,β2-MG and CRP of cerebrospinal fluid in new-onset group,relapse group were higher than those in remission groups,（190±101.91） u/L vs（132±91.12） u/L,（284±91.33） u/L vs（132±91.12） u/L（P〈0.05）;（1.19±.1.47） mg /L vs（0.72±2.10） mg /L,（1.59±1.71） mg /L vs（0.72±2.10） mg /L（P〈0.05）;（5.52±2.12） mg /L vs（2.35±3.31） mg /L,（6.90±3.29） mg /L vs（2.35±3.31） mg /L respectively（P〈0.05）. Conclusion： The levels of LDH,β2-MG and CRP of AML patients＇ serum and spinal fluid vary with the state of illness and are related with the number of tumor cells and extramedullary infiltration.It can reflect tumor load and be helpful to judge the state of illness,prognosis and rel
