三维适形放疗联合微波热疗治疗颈部淋巴结转移癌的疗效观察

摘　　要：目的:观察三维适形放疗配合微波热疗对颈部淋巴结转移性癌的治疗效果。方法:对55例来源于鼻咽癌、喉癌、食管癌及肺癌的颈部淋巴结转移性癌,分为微波热疗+三维适形放疗组30例及单纯三维适形放疗组25例,放疗总量60-70Gy/30-35次;6次/周;微波热疗3次/周,随访3个月,观察疗效。结果:两组患者全部完成治疗计划。微波热疗+三维适形放疗组患者在治疗结束时颈部肿块完全消退23例(76.7%),单纯三维适形放疗组患者为7例(35.0%),两组比较差异有统计学意义(P<0.05)。随访结束时微波热疗+三维适形放疗组患者颈部肿块完全消退25例(83.3%),单纯三维适形放疗组为15例(60.0%),两组比较差异有统计学意义(P<0.05)。无明显不良反应。结论:三维适形放疗联合微波热疗对治疗颈部淋巴结转移性癌疗效良好。Objective：To observe the efficacy to malignant tumors migrated to neck lymph-nodes treated by three dimensional conformal radiotherapy combined with microwave hyperthermia.Methods： Fifty-five patients diagnosed as nasopharyngeal carcinoma,lung cancer,larynx carcinoma,oesophagus carcinoma which have migrating to neck lymph-node were divided to three dimensional conformal radiotherapy combined with microwave hyperthermia（R＋H） group and three dimensional conformal radiotherapy（R） group.Radiotherapy DT were 60-70Gy,six times a week,microwave hyperthermia were used three times a week.All patients were followed-up for 2 months and observed the efficacy.Results： After treatment,in the R＋H group,the total subsudised rate of neck lymph-nodes was 76.7%（23/30）,whereas in R group,the total subsudised rate of neck lymph-nodes was 35.0%（7/25）（P0.05）.After follow-up for 3 months,in R＋H group,the total subsudised rate of neck lymph-nodes was 83.3%（25/30）,whereas in R group,the total subsudised rate of neck lymph-nodes was 60.0%（15/25）（P0.05）.There were no obvious side-effcets.Conclusion： Three dimensional conformal radiotherapy combined microwave hyperthermia has better efficacy than radiotherapy alone to neck lymph-nodes which malignant tumors migrated.
